, have two or more chronic diseases, and consume vegetable-based diets.
Chinese community-dwelling older adults are extensively exposed to phthalates, especially to DBP and DEHP. More attention should be paid to urban-rural differences in exposure to HMW and LMW phthalates and to phthalate exposure among older adults with overweight/obesity, females, and individuals who are current heavy smokers, have two or more chronic diseases, and consume vegetable-based diets.Tree ring records are increasingly being used as a geochemical archive of past atmospheric mercury (Hg) pollution. However, it is not clear whether all tree species can be used reliably for this purpose. We compared tree-ring Hg records of two coniferous species - widely used Scots pine (Pinus sylvestris) and less frequently used European larch (Larix decidua) at 6 study sites across the Czech Republic. Site-specific mean Hg concentrations in tree-ring segments of larch ranged from 2.1 to 5.2 μg kg-1, whereas pine had higher mean Hg concentrations (3.6-8.3 μg kg-1). Temporal records of Hg concentrations in tree rings of larch and pine differed significantly. Comparisons with previously documented peat Hg records showed that larch tree-ring Hg records more closely agreed with peat archive records. For pines, which had a large, tree-age dependent number of sapwood rings (62 ± 17, 1SD), we found a strong relationship between the year of peak Hg and the number of sapwood tree rings (p = 0.012, r2 = 0.35), as well as between peak Hg year and the sapwood-heartwood boundary year (p less then 0.001, r2 = 0.65), rather than with temporal changes in atmospheric Hg levels. The much greater number of pine sapwood tree rings appears to promote radial Hg translocation, resulting in the shift of Hg peaks backward in time through the tree-ring record. In contrast, Larch consistently had a low number of sapwood tree rings (19 ± 6, 1SD), and more closely agreed with peat Hg records. This study suggests that European larch, a tree species characterized by a relatively low and consistent number of sapwood tree rings, records changes in atmospheric Hg concentrations more reliably than does Scots pine, a species with a relatively high and variable number of sapwood tree rings.
Tuberculosis is a multisystematic disease and is the most common cause of infectious disease-related mortality worldwide. Gastrointestinal tract is an uncommon site for extrapulmonary tuberculosis (TB). Esophageal TB is exceedingly rare.
We report a 22-years-old male with esophageal TB that presented in septic shock from esophageal perforation. Despite all measures including surgical intervention and aggressive support in the intensive care unit, patient passed away.
The most common mechanism for esophageal involvement is secondary to direct spread from mediastinal structures and/or spreading the inoculation of swallowed sputum, or hematogenous or lymphatic spread. Once the diagnosis of TB is established, antibiosis is the cornerstone of treatment. Deutenzalutamide purchase Surgery is reserved only for complications of TB such as fistula, abscess, strictures or perforation. Less than 50% of cases are diagnosed within 24 h, and delay in diagnosis lead to significant increases in the mortality.
In countries with high prevalence of TB, this diagnosis should be considered in those with esophageal perforation with no underlying etiology and medical treatment for TB should be initiated in addition to conventional treatment in appropriate group of patients.

Recent advances in chemotherapy and chemoradiotherapy allow performance of conversion surgery by improving tumor shrinkage in select patients with initially unresectable locally advanced pancreatic cancer (LAPC), thereby providing curative potential. The number of conversion surgeries requiring arterial reconstruction for select patients with initially unresectable LAPC following favorable responses is expected to increase, so providing effective options for safe arterial reconstruction is critical.
Herein we report a case of successful conversion surgery for initially unresectable LAPC with splenic artery transposition for hepatic arterial reconstruction after gemcitabine/nab-paclitaxel (GnP). A 71-year-old woman was referred to our hospital for evaluation of a pancreatic head mass after developing diabetes. She was diagnosed with unresectable LAPC, which was in wide contact with the common hepatic artery (CHA), proper hepatic artery (PHA), and splenic artery (SA). She received GnP, and after 6 cycles, dact disappears due to chemotherapy.
Even for a SA initially in contact with the tumor, SA transposition for hepatic artery reconstruction is a safe and effective option when tumor contact disappears due to chemotherapy.
Celiac artery aneurysm is very rare visceral artery aneurysm. Symptomatic and ≥ 2.5 cm sized aneurysm requires treatment. Excision and revascularization is the most commonly employed procedure.
We report a case of ligation and excision of celiac artery aneurysm extending onto the splenic and hepatic arteries without vascular reconstruction. The patient was a 52 year old lady who was evaluated for abdominal pain and was found to have a celiac artery aneurysm involving the hepatic and splenic arteries. She was evaluated with computerized tomography and digital subtraction angiography of the abdominal vessels. These confirmed good natural collaterals from the branches of superior mesenteric artery supplying the liver, stomach and spleen. We performed ligation and excision of the aneurysm and ligation and division of hepatic, splenic and left gastric arteries as the aneurysm was extending on to these vessels, without any vascular reconstruction, utilizing the natural collaterals from the superior mesenteric artery.
Ligation of celiac artery aneurysm without revascularization is often done in emergency situations. Excision and revascularization is the treatment of choice to ensure adequate blood supply to liver, spleen and stomach. We could utilize the natural collateral circulation of celiac artery from superior mesenteric artery avoiding a complex procedure of revascularization.
We present this because of the rarity of the disease as well as rarity of the technique of not performing vascular reconstruction. We emphasize on the pre-operative and operative evaluation of collateral circulation with conventional angiography and intraoperative Doppler respectively.
