Background

Pragmatic trials provide real-world evidence that can be used to make clinical decisions. Practical trials should also aim to attract patients from a variety of health care settings, so that their results can be applied to the real world.

Furthermore studies that are pragmatic should focus on outcomes that are important for patients, such as quality of life or functional recovery. This is particularly important when it comes to trials that involve invasive procedures or those with potential dangerous adverse events. The CRASH trial29, for instance, focused on functional outcomes to compare a two-page report with an electronic system for monitoring of hospitalized patients with chronic heart failure. In addition, the catheter trial28 used urinary tract infections caused by catheters as its primary outcome.

In addition to these aspects, pragmatic trials should minimize the trial procedures and data collection requirements to reduce costs. Additionally pragmatic trials should try to make their findings as relevant to actual clinical practice as is possible by making sure that their primary analysis is the intention-to-treat approach (as described in CONSORT extensions for pragmatic trials).

Despite these requirements however, a large number of RCTs with features that challenge the notion of pragmatism were incorrectly labeled pragmatic and published in journals of all types. This could lead to false claims about pragmatism, and the term's use should be standardised. The development of the PRECIS-2 tool, which offers an objective and standard assessment of pragmatic features is a good initial step.

Methods

In a pragmatic study the aim is to inform clinical or policy decisions by demonstrating how an intervention could be integrated into routine care in real-world settings. This differs from explanation trials that test hypotheses regarding the cause-effect connection in idealized situations. In this way, pragmatic trials could have less internal validity than explanation studies and are more susceptible to biases in their design analysis, conduct, and design. Despite their limitations, pragmatic studies can be a valuable source of information for decision-making within the healthcare context.

The PRECIS-2 tool evaluates an RCT on 9 domains, ranging between 1 and 5 (very pragmatic). Furthermore, a common feature of pragmatic trials is that researchers attempt to make their findings more valuable by studying subgroups of the trial. However, this often leads to unbalanced comparisons and lower statistical power, thereby increasing the risk of either not detecting or misinterpreting differences in the primary outcome. This was the case in the meta-analysis of pragmatic trials due to the fact that secondary outcomes were not corrected for differences in covariates at baseline.

In addition, pragmatic studies may pose challenges to gathering and interpretation of safety data. This is due to the fact that adverse events tend to be self-reported and are susceptible to delays, inaccuracies or coding variations. It is crucial to increase the accuracy and quality of the results in these trials.

Results

While the definition of pragmatism does not require that all trials be 100% pragmatic, there are some advantages of including pragmatic elements in clinical trials. These include:

The original PRECIS tool3 included similar domains and an assessment scale ranging from 1 to 5. Koppenaal et. al10 devised an adaptation of the assessment, known as the Pragmascope which was more user-friendly to use for systematic reviews. They found that pragmatic systematic reviews had higher average scores across all domains, but lower scores in the primary analysis domain.

The difference in the primary analysis domains could be explained by the way most pragmatic trials analyse data. Some explanatory trials, however, do not. The overall score was lower for systematic reviews that were pragmatic when the domains on the organization, flexibility of delivery and follow-up were merged.

It is important to remember that the term "pragmatic trial" does not necessarily mean a low quality trial, and in fact there is an increasing rate of clinical trials (as defined by MEDLINE search, but it is neither sensitive nor specific) that use the term 'pragmatic' in their abstracts or titles. These terms may indicate that there is a greater appreciation of pragmatism in titles and abstracts, but it's not clear whether this is reflected in content.

Conclusions

In recent years, pragmatic trials have been gaining popularity in research as the value of real-world evidence is becoming increasingly acknowledged. They are randomized studies that compare real-world care alternatives to clinical trials in development. They are conducted with populations of patients that are more similar to those who receive treatment in regular care. This method is able to overcome the limitations of observational research like the biases that are associated with the reliance on volunteers, as well as the insufficient availability and the coding differences in national registry.

Other advantages of pragmatic trials include the ability to utilize existing data sources, and a higher likelihood of detecting meaningful changes than traditional trials. They discovered 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or more) in at least one of these domains.

Trials with a high pragmatism score tend to have broader eligibility criteria than traditional RCTs that have specific criteria that aren't likely to be present in the clinical environment, and they comprise patients from a wide variety of hospitals. The authors claim that these traits can make pragmatic trials more meaningful and applicable to everyday clinical practice, however they do not necessarily guarantee that a trial using a pragmatic approach is completely free of bias. Furthermore, the pragmatism of the trial is not a definite characteristic and a pragmatic trial that does not contain all the characteristics of an explanatory trial can produce reliable and relevant results.
