Subacute thyroiditis can be rarely associated with autoimmune thyroid disorders. It includes Graves' disease which is characterized by the presence of a highly specific antibody known as thyroid-stimulating hormone (TSH) receptor antibody (TRAb). There are three types of TRAb TSH receptor stimulating antibody (TSAb) which stimulates the TSH receptor causing Graves' disease, TSH receptor blocking antibody (TBAb) which blocks the TSH receptor causing hypothyroidism, and a neutralizing antibody which does not alter the thyroid function. There are two assays used to check the TRAb the thyroid-stimulating immunoglobulin (TSI) assay and the TSH receptor-binding inhibitor immunoglobulin (TBII) assay out of which the TSI assay measures the stimulating antibody which is specific for graves' disease. Although autoimmune thyroid disorders can rarely occur following subacute thyroiditis, their clinical presentation is usually compatible with the type of antibody detected in the patient's serum. We present a unique case of a 44-year-old patient who presented with subacute thyroiditis followed by the development of persistent hypothyroidism even in the presence of elevated Graves' disease-specific TSI and TRAb.IgA nephropathy (IgAN) remains one of the most common forms of glomerulonephritis, especially in developed countries with a low prevalence of infectious diseases. Tanespimycin in vivo Despite supportive measures that slow the rate of progression of chronic kidney disease (CKD) in IgAN, many patients still progress to end-stage kidney disease. Proteinuria has been shown to be an adverse prognostic factor in IgAN. Data support the use of proteinuria reduction as a reasonably likely surrogate endpoint for a treatment's effect on progression to end-stage renal disease (ESRD) in IgAN. Currently employed immunosuppressive strategies lack conclusive efficacy data, while there is evidence for treatment-induced toxicity. The current standard of care for the management of IgAN is intensive goal-directed supportive care. Recently the role of sodium-glucose cotransporter 2 (SGLT2) inhibitors in decreasing proteinuria and progression of CKD is widely being recognized. In this case report, we present a 44-year-old male with proteinuria and biopsy-proven IgAN who remained in remission after six months of steroids using the Pozzi protocol. He developed proteinuria five years after remission. At this point, canagliflozin was added to his angiotensin-receptor blocker (ARB) therapy resulting in a significant reduction in his proteinuria. Our case report may intrigue researchers to look into the role of canagliflozin in decreasing albuminuria in non-diabetic kidney disease, thus slowing the progression to ESRD.
Immunoglobulin A nephropathy (IgAN) is the most common primary aetiology of glomerulonephritis worldwide, and it is the most important type in terms of morbidity and mortality. IgAN involves the deposition of immune bodies in the mesangial cells of the kidney, which causes renal glomerular damage, haematuria, proteinuria, and various other symptoms. Previous studies have mainly focused on the East Asian population, with few studies conducted in Saudi Arabia, particularly in the western region. The diagnosis requires a biopsy, which should be examined by an expert and classified according to the Oxford classification system.
Analyze the clinical, pathological, and laboratory features of male and female patients diagnosed with IgAN at King Abdulaziz University Hospital (KAUH).
This was a retrospective record review conducted at KAUH in Jeddah, Saudi Arabia between May-June 2021. The study included 18 patients diagnosed with IgAN by biopsy, and their clinical, laboratory and pathological data were evaluate classification in their kidney biopsies compared to women.Conjoined twins represent a very rare congenital anomaly, and the dicephalic dibrachius dipus (DDD) type of conjoined twinning is so rare that the exact prevalence is unknown. Only a few published case studies have mentioned this anomaly. Not enough data are available where antenatal ultrasonography (USG) and MRI have been employed in the workup of such cases. This study describes the case of a 24-year-old woman who came to our department for an anomaly scan at 25 weeks of gestation and was diagnosed with a dicephalic type of conjoined twinning with multiple anomalies. However, USG could not differentiate between DDD twinning and craniopagus parasiticus; hence, the patient was referred for fetal MRI. On MRI, the diagnosis of DDD was confirmed. In craniopagus parasiticus twinning, the surgical removal of the parasitic head can allow an everyday life. However, DDD twinning with multiple anomalies is not compatible with life, and the mother was thoroughly explained the grave prognosis. In such doubtful cases, fetal MRI should always be employed to ascertain the diagnosis for proper management and counseling.Pheochromocytoma and paraganglioma are neuroendocrine tumors that occur less commonly among children compared to adults. The excess catecholamines secreted by the tumor cells result in hypertension, tachycardia, excess sweating, and headache. Other symptoms include abdominal pain or distension caused by the adrenal mass. Here, we report a case of pheochromocytoma arising from the left adrenal medulla in a 14-year-old boy, which was exclusively secreting norepinephrine, as suggested by elevated plasma and 24-hour urinary norepinephrine and its metabolite normetanephrine. The epinephrine and its metabolite metanephrine were within normal limits. He presented with abdominal pain, recurrent vomiting, and headache and was noted to have elevated blood pressure. He underwent adrenalectomy after controlling his blood pressure with an alpha-blocker Prazosin. His blood pressure remained stable after surgery, and his plasma-free metanephrines returned to normal limits. He tested negative for hereditary paraganglioma-pheochromocytoma gene panel.Natural deep eutectic solvents (NADESs) are considered as green solvents, and due to their promising sustainability, they have been applied in many research fields. In this study, the main goal is to use various NADES systems to replace the traditional solvents used in conservation and restoration to remove varnish layers in a painting. The toxicity of traditional solvents, such as toluene or acetone, is well known in the chemistry field. To replace them, it is important to understand the intrinsic physicochemical properties of a solvent that may act as a substitute. Polarity and solubility are proposed as the best parameters required for this study. The Nile red probe was used to confirm the similarity between the polarity of deep eutectic systems (DESs) and traditional solvents. According to their polarities and Hansen solubility parameters, it is possible to predict the best solvents to solubilize the natural resin varnishes. Besides this, some arithmetic models can also be applied to estimate the critical or thermodynamic properties, which are useful tools to predict the behavior of these solvents. We have further proven the possibility of dissolving natural varnishes such as dammar or mastic in hydrophobic DESs, such as menthol + lauric acid, menthol + decanoic acid, or menthol + thymol.In cancer patients, the clinical response to checkpoint-based immunotherapy is associated with the composition and functional quality of the host microbiome. While the relevance of the gut microbiome for checkpoint immunotherapy outcome has been addressed intensively, data on the role of the local tumor microbiome are missing. Here, we set out to molecularly characterize the local non-small cell lung cancer microbiome using 16S rRNA gene amplicon sequencing of bronchoscopic tumor biopsies from patients treated with PD-1/PD-L1-targeted checkpoint inhibitors. Our analyses showed significant diversity of the tumor microbiome with high proportions of Firmicutes, Bacteroidetes and Proteobacteria. Correlations with clinical data revealed that high microbial diversity was associated with improved patient survival irrespective of radiology-based treatment response. Moreover, we found that the presence of Gammaproteobacteria correlated with low PD-L1 expression and poor response to checkpoint-based immunotherapy, translating into poor survival. Our study suggests novel microbiome-specific/derived biomarkers for checkpoint immunotherapy response prediction and prognosis in lung cancer. In a broader sense, our data draw attention to the local tumor microbial habitat as an important addition to the spatially separated microbiome of the gut compartment.The COVID-19 pandemic has significantly changed the way we treat patients and educate healthcare professionals (HCPs). In summer 2020, the International League against Epilepsy (ILAE) implemented a virtual CME program with three integrated program elements addressing challenges in patient treatment as well as challenges caused by the forced transition to a virtual environment. Despite the highly competitive environment with exponential increase of webinars offered to HCPs, the program achieved high participation and satisfaction rates. Over 60% of participants indicated a change in their clinical practice after the interventions. With our outcomes evaluation, we aimed to better understand how well such an integrated program resonates with the learner and if it can make a difference in a highly competitive environment by supporting educators to become more adaptive and responsive to learner needs. Our pilot project was shown to be well accepted, achieving high satisfaction and perceived impact by the learner. In the light of an upcoming "digital fatigue" and a wish to return to face-to-face, we reiterate the value of the digital approach and recommend continuing along this successful path as we believe that taking a learner on a digital educational journey has been successful in a highly competitive and challenging environment.The - possibly - imminent AI revolution will, to a great extent, affect the education and training for all knowledge-working professions, and therefore must be considered an important aspect also of CME. These new skills should be reflected in the future curricula of medical faculties, in particular during specialist medical education, and integrated into continuous professional development (CPD) to keep doctors up-to-date with digital innovations.Physicians and patients are increasingly represented on social media and perform as so-called medical influencers. As key opinion leaders, they can influence and change opinions on health topics and also establish their work within a specialist community.
