RAS on the cytoplasmic side of the plasma membrane is activated by SOS through the recruitment of GRB2/SOS complex to the RTKs that are phosphorylated after stimulation with growth factors. The adaptor protein GRB2 recognizes phospho-RTKs both directly and indirectly via another adaptor protein, SHC. We here studied the regulation of GRB2 recruitment under the SHC pathway using single-molecule imaging and fluorescence correlation spectroscopy in living cells. We stimulated MCF7 cells with a differentiation factor, heregulin, and observed the translocation, complex formation, and phosphorylation of cell signaling molecules including GRB2 and SHC. Our results suggest a biphasic regulation of the GRB2/SOS-RAS pathway by SHC At the early stage ( less then 10 min) of stimulation, SHC increased the amplitude of RAS activity by increasing the association sites for the GRB2/SOS complex on the plasma membrane. CD has been associated with gastrointestinal malignancies, most commonly gastrointestinal lymphoma. Rare malignancies have also been reported, such as small intestinal adenocarcinoma. In this report, we present a case of a 91-year-old male with a history of CD, noncompliant with a gluten-free diet, who presented with weight loss, abdominal pain, and gastrointestinal bleeding secondary to a newly discovered adenocarcinoma of the jejunum.The innominate artery aneurysm (IAA) accounts for a small percentage of all peripheral aneurysms. However, its clinical outcomes are potentially devastating, especially when it is associated with coronary disease, due to the high risk of spontaneous rupture and thromboembolic complications. Surgical repair is always recommended in such cases. The treatment of such a condition presents a surgical challenge with high morbidity and mortality rates. In this report, we discuss the case of a 56-year-old male who presented with a right cervical mass secondary to a large IAA with underlying coronary artery disease. The patient underwent a simultaneous operation for IAA and coronary bypass grafting.Cardiac implantable electronic device (CIED) infections are a serious complication of both initial device implants and generator change procedures, and they are associated with a wide range of presentations. Reported rates of CIED infections vary widely from 0.1% to 19.9%, but it is estimated that they occur in 0.5% of initial device implants and 1-7% of subsequent implants. It is widely accepted that the administration of local antibiotics within the pocket as well as extracellular matrices (ECMs) can be utilized to reduce the incidence of CIED infections. We describe a case where the use of an additional biological ECM scaffold sutured directly into the incision site was utilized in addition to a biological ECM pouch in order to reduce the risk of infection. We propose that biological ECM could be utilized to reinforce the incision site directly as well as ECM within the pocket to reduce the instances of CIED infections. Further investigation of the use of biological ECM to prevent infection is warranted and paramount to further decrease the number of complications associated with device implantation.Introduction Electronic Health Record (EHR) adoption rates for office-based physicians doubled between 2008 and 2015, from 42% to 89%, and more than 60% of all office-based physicians achieved meaningful use by 2016. The US government has paid billions of dollars in incentives to promote EHR meaningful use. Nonetheless, evidence linking EHR meaningful use to quality measures improvements is limited. Objective This study aims to examine the relationship between EHR meaningful use and capabilities among four quality measures in an ambulatory healthcare setting. Study design A cross-sectional study design of the 2015-2016 National Ambulatory Medical Care Survey dataset. Methods We used adjusted multivariate regression models to examine associations between (a) EHR meaningful use and (b) 10 EHR-computerized capabilities, with four quality measures (blood pressure screening, tobacco use screening, obesity screening, and obesity education). Results We analyzed 30,787 office visits, representing an annual estimate of 680 million national office visits.
