Gastric varices (GV) are an important complication of portal hypertension, and the current recommendation for management is beta-blocker therapy for primary prophylaxis and transjugular intrahepatic portosystemic shunt (TIPS) for active bleeding or secondary prophylaxis. Direct endoscopic injection of cyanoacrylate (CYA) glue has been investigated but has drawbacks including limited endoscopic characterization of GV and possible distal glue embolism. To this end, endoscopic ultrasound (EUS) has been pursued to help in characterization of GV, visualization of treatment in real time, and confirmation of obliteration with Doppler.

In this paper, we review treatments for GV involving EUS, including EUS-guided injection of CYA and coils, either alone or in combination. We also discuss less common methods, including EUS-guided injection of thrombin and absorbable gelatin sponge. We then review literature comparing EUS-guided methods with direct endoscopic therapy and comparing individual EUS-guided methods withover conventional therapy, lack of a standardized approach, and limited availability of expertise and necessary materials. Novel EUS-based methods offer a unique opportunity to directly visualize and access gastric varices for treatment and obliteration. This may provide key advantages over current endoscopic or angiographic treatments. Comparative studies investigating the benefit of EUS over conventional therapy are needed.The monitoring of the ciliated walls in the uterine tube has supreme importance in enhancing the sperm to reach the egg (capacitation processes), and at peristaltic ciliary flow has a more favorable residual time along the canal when compared to the peristaltic flow. eFT508 Based on the importance of this study, a mathematical simulation of this process has been carried out by studying the behavior of a non-Newtonian magnetized fluid with a Darcy flow model with an oscillating wall having an internal ciliated surface. The governing equation is formed with Eyring-Powell fluid (tubal fallopian fluid) without using any approximations and solved using the Adomian analysis method. Using the vorticity formula, the components of the velocity function, pressure gradient, and stream function are obtained. The influence of relevant parameters is explained through diagramming and discussion. We also analyzed the residue time effects on the flow parameters. The results indicate that peristaltic ciliary flow has a more favorable residual time along the canal when compared to peristaltic flow.
Gastroesophageal reflux disease (GERD) is a common disease encountered in daily medical care and clinical problem which hampers daily life and reduces quality of life (QOL). The coexistence of GERD-related symptoms with the typical GERD symptoms, such as heartburn or acid regurgitation, and various upper abdominal symptoms is frequently observed in patients with GERD. However, the effect of these coexisting symptoms on the daily life and QOL of patients with GERD has not been clarified. Therefore, the effects of the various upper abdominal symptoms coexisting with GERD on the daily life and QOL of such patients were compared.

A total of 113 newly diagnosed patients who visited our hospital with typical GERD symptoms were assessed using the modified frequency scale for the symptoms of GERD (MFSSG), gastroesophageal reflux and dyspepsia therapeutic efficacy and satisfaction test (GERD-TEST), and short-form 8-item health survey (SF-8) questionnaires. The "gastroesophageal reflux symptom" (7 items) and "dyspel items in SF-8 (PCS, MCS) (P < 0.05). Multiple regression analysis indicated the largest influences of each symptom category on living status and QOL in descending order dissatisfaction for eating (atypical FD, typical FD), daily activities (atypical FD, typical FD, typical GERD), mood (atypical FD), daily life-SS (atypical FD, typical FD), PCS (typical FD), and MCS (atypical FD) (P < 0.05).

The yeast Pichia pastoris has shown a great biotechnological potential for producing heterologous proteins and renewable chemicals. Despite its relevance, the performance of P. pastoris in presence of lignocellulose-derived inhibitors remains unclear. In this work, our results show for the first time the dose-dependent response of P. pastoris to acetic acid, furaldehydes (HMF and furfural), and sugarcane biomass hydrolysate, both at physiological and transcriptional levels. The yeast was able to grow in synthetic media with up to 6 g.L-1 acetic acid, 1.75 g.L-1 furaldehydes or hydrolysate diluted to 10% (v/v). However, its metabolism was completely hindered in presence of hydrolysate diluted to 30% (v/v). Additionally, the yeast was capable to co-consume acetic acid and glucose. At the transcriptional level, P. pastoris response to lignocellulose-derived inhibitors relays on the up-regulation of genes related to transmembrane transport, oxidoreductase activities, RNA processing, and the repression of pathways related to biosynthetic processes and central carbon metabolism. These results demonstrate a polygenetic response that involves detoxification activities, and maintenance of energy and cellular homeostasis. In this context, ALD4, OYE3, QOR2, NTL100, YCT1, and PPR1 were identified as target genes to improve P. pastoris tolerance. Altogether, this work provides valuable insights into the P. pastoris stress tolerance, which can be useful to expand its use in different bioprocesses.A simple and feasible pH meter-based immunoassay is reported for detection of C-reactive protein (CRP) using glucose oxidase (GOD)-conjugated dendrimer loaded with platinum nanozyme. Initially, platinum nanozymes were loaded into the dendrimers through an in situ synthetic method. Then, GOD and monoclonal anti-CRP antibody with a high molar ratio were covalently conjugated onto carboxylated dendrimers via typical carbodiimide coupling. The immunoreaction was carried out with a competitive mode in a CRP-coated microplate. Along with formation of immunocomplex, the added glucose was oxidized into gluconic acid and hydrogen peroxide by GOD, and the latter was further decomposed by platinum nanozyme, thus accelerating chemical reaction in the positive direction. The produced gluconic acid changed the pH of detection solution, which was determined using a handheld pH meter. Under optimum conditions, the pH meter-based immunoassay gave a good signal toward target CRP from 0.01 to 100 ng mL-1. The limit of detection was 5.9 pg mL-1. An intermediate precision ≤ 11.2% was acquired with batch-to-batch identification. No nonspecific adsorption was observed during a series of procedures to detect target CRP, and the cross-reaction against other biomarkers was very low. Importantly, our system gave well-matched results for analysis of human serum samples relative to a referenced ELISA kit.Graphical abstract.The present study was conducted to compare the growth performance, feed efficiency, nitrogen retention, water quality, biofloc nutritional composition, and hematological profile of Nile tilapia Oreochromis niloticus fingerlings cultured in biofloc technology (BFT) and recirculating aquaculture systems (RAS) over a period of 60 days. This randomized study was designed with two treatments (BFT and RAS) and four replicates. Fish (n = 64; 7.29 ± 0.67) were allocated in eight 150 L aquaria kept at 26.72 ± 0.35 °C and fed until apparent satiation three times daily. Weight gain, apparent feed conversion ratio, and protein efficiency of fish in BFT were statistically superior than in RAS. There were no differences (p > 0.05) for temperature, dissolved oxygen, and nitrogen compounds, indicating similarity in the water quality between BFT and RAS. Furthermore, hematological parameters did not show differences (p > 0.05) between the systems. link2 The chemical composition of biofloc differed (p  less then  0.05) over the experimental period for crude protein and mineral matter. The best responses of growth performance and feed efficiency of Nile tilapia fingerlings in BFT highlight the importance of bioflocs as a high biological value complementary feed.Soil heavy metal (Cr, Cu, Zn, Pb, Cd, V, As) concentrations in different areas were analyzed to investigate the effects of different industrial activities on heavy metal pollution status, potential ecological risk, and human health risk in Panzhihua. Our results showed that Cu and V enrichment in soil was due to ore smelting. Soil Cr accumulation was related to coal ore mining. Soil Cd, Zn, As, and Pb enrichment was attributed to high-temperature coal combustion. Under the effect of industrial activities, soils were moderately contaminated with Cd, uncontaminated to moderately contaminated with As and Zn, and uncontaminated with Cr, Cu, V, and Pb. Soil heavy metal potential ecological risk was considerable, and non-carcinogenic risks and carcinogenic risks of soil heavy metals were acceptable for adults but unacceptable for children. Thermal power generation was the dominated industrial activity that influence the soil heavy metal concentrations and environmental risks in Panzhihua, which posed considerable potential ecological risks and unacceptable heavy metal non-carcinogenic risks and As carcinogenic risk to both adults and children. This study indicates that industrial activities have great effects on heavy metal pollution, ecological risks, and health risk, and more attention should be paid to the ecological risk and health risks brought by thermal power generation.Despite a variety of definitions of mindfulness, over the past 20 years there have been increasing claims that mindful practice is helpful in improving the accuracy of clinical diagnosis. We performed a systematic review and evidence synthesis in order to determine the nature and definitions of mindful practice and associated terms; evaluate the quality of evidence for the benefits of mindful practice; and conclude whether mindful practice may reduce diagnostic error. We screened 14397 refereed reports from the five common literature databases, to include 33 reports related to the use of mindful practice in clinical diagnosis. Our evidence synthesis contained no randomised controlled trials (level I evidence) of mindful practice, the majority of supporting evidence (26 reports or 79%) comprised conceptual commentary or opinion (level IV evidence). link3 However, 2 supporting reports constituted controlled studies without randomisation (level IIa), 1 report was quasi-experimental (level IIb), and 4 reports were comparative studies (level III).
