Antimicrobial, antiviral, antioxidant, anticancer, anti-inflammatory, antimutagenic, antidiabetic, nephroprotective, gastroprotective, hepatoprotective, and cardioprotective activities were all demonstrated in T. cordifolia stem extracts. To validate pharmacodynamic targets, further research is needed to evaluate the molecular mechanisms of the known compounds against gastrointestinal diseases, inflammatory processes, and microbial infections, as immunostimulants, and in chemotherapy. The T. cordifolia safety profile was confirmed in a toxicological analysis, which prompted pharmacokinetic assessment testing to confirm its bioavailability.
Dengue virus (DENV) is a re-emerging mosquito-borne flavivirus that has recently engendered large epidemics around the world. Consequently antivirals with effective anti-DENV therapeutic activity are urgently required. In the 18th century, Europeans, as well as native inhabitants of North America, were known to adapt the medicinal property of the common perennial plant Eupatorium perfoliatum L. to treat fever and infections. Previous studies have shown that Eupatorium perfoliatum L. possesses anti-inflammatory, anti-oxidative, anti-plasmodial, anti-bacterial and antiviral activities. However, to the best of our knowledge, no anti-DENV activity of E. perfoliatum L. has been investigated at the molecular level so far.
Here, for the first time we have attempted to study the action of E. perfoliatum extract and its few bioactive components i.e., quercetin, caffeic acid and eupafolin against wild primary clinical isolate of DENV-2 infection in an in vitro model.
The presence of the bioactive components in thponents, in parallel to earlier studies, quercetin showed the most significant preventive action against DENV infection. Tanespimycin Further, in molecular docking analysis also, quercetin showed the strongest binding affinity towards DENV membrane receptor TIM-1 protein.
Our findings suggests that E. perfoliatum extract has significant potential to be an anti-DENV therapeutic agent. Moreover, among the bioactive components, quercetin may have a prophylaxis role in executing the antiviral activity of E. perfoliatum extract against DENV infection.

Currently, some photoreactivity assays such as UV-VIS spectral analysis and ROS assay are generally used for initial photosafety assessments because of their high sensitivity. However, they have a low specificity, generating a high percentage of false positive results, and the development of a follow-up assessment method is desired. Therefore, this study aimed to develop an in chemico photoallergy testing method, photo-direct peptide reactivity assay (photo-DPRA). Based on photosafety information, 34 photoallergens and 16 non-photoallergens were selected and subjected to UV-VIS spectral analysis, ROS/micellar ROS assays, photo-DPRA, sequential testing strategy (STS) consisting of all three methods, and 3T3 neutral red uptake phototoxicity testing (3T3 NRU PT). Combination of the methods addressing the key events of photoallergy exhibited high prediction performance. Our results showed the proposed strategy would be useful to predict the photoallergic potential of chemicals as the follow-up assessment for false positive chemicals by UV/VIS spectral analysis and ROS assay.Haloarchaea are adapted to survive under extreme saline conditions by accumulating osmolytes and salts to counteract the high osmotic pressure in their habitats. In the adult population of Alberta, we examined 3 cohorts 1) Probable cases of cardiac amyloidosis the presence of physician-assigned diagnosis of amyloidosis (ICD-10 E85; ICD-9 277.3) and ≥1 healthcare encounter for heart failure (HF) (ICD-10 I50; ICD-9 428); 2) Possible cardiac amyloidosis the presence of clinical phenotypes suggestive of amyloidosis; and 3) Comparator HF cohort without amyloidosis.
Between 2004 and 2018, 982 of the 145,329 patients with HF were identified as probable cardiac amyloidosis. During the same period, the incidence rates of probable cardiac amyloidosis increased from 1.38 to 3.69 per 100,000 person-years and the prevalence rates increased from 3.42 to 14.85 per 100,000 person-years (p
<0.0001). Patients with probable cardiac amyloidosis were more likely to be male, have higher comorbidity burden, greater healthcare utilization and poorer outcomes as compared to HF patients without amyloidosis. A much larger group of patients was identified as possible cardiac amyloidosis (n=46,255), with similar increase in prevalence from 2004 to 2018 (from 416 to 850 per 100,000 person-years).

We conducted a nationally-representative, retrospective cohort study of patients with previous bariatric surgery undergoing either heart transplantation or left ventricular assist device implantation. Of 200 patients, less then 6% experienced in-hospital mortality after destination therapy, comparable to that reported in the general heart recipient population. Risk-adjusted outcomes minimally differed from obese patients undergoing destination therapy without previous bariatric surgery. This study provides important safety benchmarking data and demonstrates the feasibility of bariatric surgery as a potential 'bridge' left ventricular assist device or transplantation in obese patients with end-stage heart failure.
Right ventricular dysfunction (RVD) is a major issue in patients with advanced heart failure (HF) since it precludes the implantation of left ventricular assist device (LVAD) usually leaving heart transplantation (HTx) as the only available treatment option. Pulmonary artery pulsatility index (PAPi) is a hemodynamic parameter integrating information of right ventricular function and of pulmonary circulation. Our aim is to evaluate the association of pre-operative RVD, hemodynamically defined as a low PAPi, with post- HTx survival.
Consecutive adult HTx recipient at two Italian transplant centers between 2000 and 2018 with available data on pre-HTx right heart catheterization were retrospectively included. RVD was defined as a value of PAPi lower than the 25
percentile of the study population. The association of RVD with 1-year post-HTx mortality and other secondary endpoint was evaluated. Multivariate logistic regression was used to adjust for clinical and hemodynamic variables. Analyses stratified by peters.
Pre-operative RVD, evaluated through PAPi, is associated with mortality and morbidity after HTx, providing incremental prognostic value over traditional clinical and hemodynamic parameters.
Medical assistance in dying (MAiD) is available in Canada for patients with grievous and irremediable medical conditions causing unbearable physical or mental suffering. It is not known how the 'grievous and irremediable suffering' criteria is being interpreted and documented by physicians.
Retrospective chart review of MAiD assessments from patients who submitted written MAiD requests to The Ottawa Hospital from June 1, 2016 to September 18, 2018. We used inductive thematic analysis to determine themes and subthemes.
Our sample included 52 patients with a mean age of 70.5 years (SD 14), 24/52 (46%) were male. We identified 5 themes 1) patient's context and history (e.g. past experiences, lack of disease modifying treatments), 2) physical symptoms (e.g. chronic pain, fatigue), 3) psychosocial symptoms (e.g. social isolation, or inability to communicate), 4) sense of control and 5) irreversibility. These themes were used to create a framework that describes the suffering of patients requesting MAiD. Patients who request MAiD describe how their disease causes suffering through physical symptoms, psychological symptoms and loss of control that is irreversible. These domains of suffering interact with their personal history and context leading to a reality that is unacceptable and irreversible.
MAiD assessors' working definition of 'grievous and irremediable suffering' as documented in their assessments is consistent with the body of literature on this topic. MAiD assessments could be enhanced with more information about existential aspects of suffering and the impact of illness on meaningful life roles.
