The objective of the present retrospective study was analysis of clinical, radiological, and electrophysiological characteristics of the non-lesional late-onset epilepsy (NLLOE) in the elderly Japanese patients, and comparison of the seizure outcomes in this population with regard to presence of comorbid dementia. The study cohort comprised 89 consecutive patients with NLLOE aged ≥ 65 years. In 49 cases (55%), NLLOE manifested with a single type of seizure. Focal impaired awareness seizures (FIAS) were encountered most often (in 69 patients; 78%). Ten patients (11%) had a history of the status epilepticus. Comorbid dementia was diagnosed in 31 patients (35%). Localized or diffuse white matter hyperintensity was the most common imaging finding (66 cases). Epileptiform discharges in the temporal area represented the most frequent abnormality on interictal EEG (24 cases). Seizure-free status for ≥ 12 months was attained in 46 out of 64 patients (72%), who were followed for ≥ 12 months (range, 12 - 110 months), and 42 of them received monotherapy, mainly with levetiracetam (21 patients), carbamazepine (10 patients), or lacosamide (8 patients). In comparison to their counterparts, the rate of seizure-free status for ≥ 12 months was significantly lower in patients with comorbid dementia (81% vs. 52%; P = 0.0205). In conclusion, the NLLOE among Japanese patients aged ≥ 65 years has variable presenting characteristics, and comorbid dementia is diagnosed in one-third of cases. Seizure-free status for ≥ 12 months may be attained in more than two-thirds of treated patients, but comorbid dementia is associated with significantly worse response to antiseizure therapy.Since about 2000 (Bowyer et al., 1998), radioxenon monitoring systems have been under development and testing for the verification of the Comprehensive Nuclear Test-Ban Treaty (CTBT). Operation of the systems since then has resulted in development of a next-generation of systems that are nearly ready for operational deployment. By 2010, the need to screen out civilian sources was well known (Auer et al., 2010; Saey, 2009), and isotopic ratio approaches were soon considered (Kalinowski and Pistner, 2006) to identify specific sources. New generation systems are expected to improve the ability to verify the absence of nuclear tests by using isotopic ratios when multiple isotopes are detected. In this work, thousands of releases were simulated to compute the global detection probability of 131mXe, 133mXe, 133Xe, and 135Xe at 39 noble gas systems in the International Monitoring System (IMS) for both current and next-generation systems. Three release scenarios are defined at 1 h, 1 d, and 10 d past a 1 kt TNT equive should increase detection confidence, tighten release location estimates, improve rejection of civilian signals, and lessen the impacts from individual systems being offline for maintenance or repair reasons.
Patient-reported outcome measures [PROMs] are key to documenting outcomes that matter most to patients and are increasingly important to commissioners of health care seeking value. We report the first series of the ICHOM Standard Set for Inflammatory Bowel Disease [IBD].
Patients treated for ulcerative colitis [UC] or Crohn's disease [CD] in our centre were offered enrolment into the web-based TrueColours-IBD programme. Through this programme, e-mail prompts linking to validated questionnaires were sent for symptoms, quality of life, and ICHOM IBD outcomes.
The first 1299 consecutive patients enrolled [779 UC, 520 CD] were studied with median 270 days of follow-up (interquartile range [IQR] 116, 504). 671 [52%] were female, mean age 42 years (standard deviation [sd] 16), mean body mass index [BMI] 26 [sd 5.3]. At registration, 483 [37%] were using advanced therapies. Median adherence to fortnightly quality of life reporting and quarterly outcomes was 100% [IQR 48, 100%] and 100% [IQR 75, 100%], respectively. learn more In the previous 12 months, prednisolone use was reported by 229 [29%] patients with UC vs 81 [16%] with CD, p <0.001; 202 [16%] for <3 months; and 108 [8%] for >3 months. An IBD-related intervention was reported by 174 [13%] patients, and 80 [6%] reported an unplanned hospital admission. There were high rates of fatigue [50%] and mood disturbance [23%].
Outcomes reported by patients illustrate the scale of the therapeutic deficit in current care. Proof of principle is demonstrated that PROM data can be collected continuously with little burden on health care professionals. This may become a metric for quality improvement programmes or to compare outcomes.

Autism spectrum disorder (ASD) is a group of neurodevelopmental conditions associated with quantitative differences in cortical and subcortical brain morphometry. Qualitative assessment of brain morphology provides complementary information on the possible underlying neurobiology. Studies of neuroradiological findings in ASD have rendered mixed results, and await robust replication in a sizable and independent sample.
We systematically and comprehensively assessed neuroradiological findings in a large cohort of participants with ASD and age-matched controls (total N=620, 348 ASD and 272 controls), including 70 participants with intellectual disability (47 ASD, 23 controls). We developed a comprehensive scoring system, augmented by standardized biometric measures.
There was a higher incidence of neuroradiological findings in individuals with ASD (89.4%) compared to controls (83.8%, p= .042). Certain findings were also more common in ASD, in particular opercular abnormalities (OR 1.9, 95% CI 1.3-3.6) and ions, and are potentially valuable to fundamental ASD research.
There was a subtly higher prevalence of neuroradiological findings in ASD, which did not appear to be specific to the condition. Individual findings or clusters of findings may point towards the neurodevelopmental mechanisms involved in individual cases. As such, clinical MRI assessments may be useful to guide further etiopathological (genetic) investigations, and are potentially valuable to fundamental ASD research. This paper presents a method to solve a linear regression problem subject to grouplassoand ridge penalisation when the model has a Kronecker structure. This model was developed to solve the inverse problem of electrocardiography using sparse signal representation over a redundant dictionary or frame. The optimisation algorithm was performed using the block coordinate descent and proximal gradient descent methods. The explicit computation of the underlying Kronecker structure in the regression was avoided, reducing space and temporal complexity. We developed an algorithm that supports the use of arbitrary dictionaries to obtain solutions and allows a flexible group distribution.
Stress in children remains a complex concept to examine due to the inherent subjectivity and lack of specific manifestations, as well as the multiple ways stress can be defined and measured in children. Because stress is multifactorial,is experienced daily by children, and undergirds adolescent health and early mental illness, it is crucial to have a clear understanding of stress and the effects of stress in children from infancy through age twelve years.
To be included in this review, literature must pertain to and highlight theories, definitions/classifications, and measurements of stress in children from infancy to 12 years of age.
The most pertinent articles identified through database searches (PubMed, Scopus, PsycINFO, CINAHL, Google Scholar), gray literature sources (e.g., child health websites), and reference lists of identified articles were included in this narrative overview.
The results of this review are organized by themes and include classifications and definitions of stress, stress-related theories, and tools to measure stress in children.
Research addressing stressors and stress in children is limited, and there is wide variation in how researchers define and classify stress in children. Existing measures of stress in children younger than 12 address physiological, psychological, and observational components, but may be inconsistent and threaten validity of otherwise well-designed and well-executed studies.
Improving the understanding and accurate measurement of stress in children enables researchers and clinicians to curtail undesirable health outcomes.

It is also the state of an animal in its attempt to cope with its environment. Animal welfare has high ethics and economic importance. Thus the need to develop parameters for assessing animal welfare. An acute increase in glucocorticoid (GC) concentration is necessary for adaptation to a stressful situation. Glucocorticoids also play a significant role in metabolic, cardiovascular, and immune systems. Glucocorticoid enhances effective learning through the hippocampus and other normal body functions. That is why we remember events (either positive or negative) associated with strong emotions. Long-term secretion of GCs has catabolic effects. Thus, affecting animal health. Measuring GC is one of the ways of assessing animal welfare. But, high GC concentration does not only indicate pain or suffering. We report that stress and emotion trigger similar physiological responses. So, measuring GC levels cannot differentiate between positive and negative states. We conclude that GC shows circadian rhythms and episodic spikes in some species. Values from a single sample point are not reliable to make conclusions about a condition. Training animals for blood collection may reduce stress. Thus not causing bias in the GC concentration measured.Exosomes participate in intercellular communication by shuttling various small molecules from donor to recipient cells. We aimed to examine the role of exosomes and exosomal miRNAs in influenza virus infection. The results showed that influenza A/H1N1pdm09 infection could promote A549 cells to secrete exosomes, while blocking the generation of exosomes reduced viral RNA production. A total of 97 exosomal miRNAs with significantly altered expression were identified during influenza infection. Of 12 candidate miRNAs chosen for further validation, ten were confirmed by qRT-PCR. Among 5978 predicted target genes,we found 37 interferon pathway-related genes to be the potential targets of 29 differentially expressed miRNAs. Many target genes were annotated to various KEGG signaling pathways, some of which played important roles in influenza infection. These data will help to further understand the mechanism of influenza virus-host interactions, which is important for the development of preventative and therapeutic strategies against influenza virus.
