STUDY OBJECTIVES To evaluate the long-term impact of laparoscopic excision of endometriosis on quality of life, through pain reduction, as measured by the Endometriosis Health Profile-30 (EHP-30), in uterine-sparing (preservation of the uterus and at least one ovary) and non-uterine sparing (removal of the uterus) surgery. DESIGN Cohort study. SETTING Academic medical center. PATIENTS 61 women who had undergone laparoscopic excision of endometriosis for pelvic pain and were enrolled in a tissue procurement study. INTERVENTIONS Patients who had previously completed an EHP-30 preoperatively and at 4-weeks postoperatively were mailed a copy of the EHP-30 2.6-6.8 years after their index surgery. MEASUREMENTS AND MAIN RESULTS The primary outcome was quality of life as measured by changes in the EHP-30 prior to their index surgery compared to weeks and years later. Secondary outcome was a comparison of the EHP-30 between patients who underwent excision of endometriosis alone versus excision of endometriosis with hyscuss during counseling for treatment of endometriosis. In this regard, an insight into the structure-immunogenicity relationships of these antigens is important but this has not been systematically investigated yet. selleck products In the current study, Lex, Ley, and KH-1 antigens with a lactose unit at the reducing end as a spacer were synthesized and coupled with keyhole limpet hemocyanin (KLH) protein. Immunological evaluations of the resultant conjugates revealed that they all could elicit robust immune responses whilst the Ley conjugate could provoke the highest titers of total and IgG antibodies. The binding assays of their antisera to each antigen and to cancer cells showed that each antiserum had extensive cross-reaction with all three antigens as protein conjugates and strong but somewhat antigen-selective binding towards MCF-7 cancer cell. Moreover, none of these antisera had obvious binding to SKMEL-28 cancer cell that does not express Lex, Ley and KH-1. The results of assays of these antisera to mediate complement-dependent cytotoxicity (CDC) to MCF-7 and SKMEL-28 cancer cells were very similar to the results of binding assays. Thus, it was concluded that all three antigens could form effective conjugate vaccines whereas the Ley conjugate induced the most robust immune responses and the antiserum of Lex had the highest binding and cytotoxicity to target cancer cells. In addition, as the antibodies induced by each antigen had extensive cross-reaction with other two antigens, either Lex or Ley or the two combined can be utilized to formulate effective conjugate vaccines for cancer immunotherapy. Another paradigm-shifting discovery of this study is that the presentation of Lex, Ley, and KH-1 antigens on cancer cell can be different from that in synthetic conjugates, which should be taken into consideration during the design and optimization of related cancer vaccines or immunotherapies. OBJECTIVE Maternal high-fat diet (HFD) has been shown to promote the development of insulin resistance (IR) in adult offspring; however, the underlying mechanisms remain unclear. METHODS Eight-week-old female wild-type mice (C57BL/6) were fed either an HFD or a normal diet (ND), one week prior to mating, and the diet was continued throughout gestation and lactation. Eight-week-old male offspring of both groups were fed an HFD for 8 weeks. RESULTS Offspring of HFD-fed dams (O-HFD) exhibited significantly impaired insulin sensitivity compared with offspring of ND-fed dams (O-ND). The adipocyte size of the eWAT increased significantly in O-HFD and was accompanied by abundant crown-like structures (CLS), as well as a higher concentration of interleukin 1β (IL-1β) in the eWAT. Treatment with an inflammasome inhibitor, MCC950, completely abrogated the enhanced IR in O-HFD. However, ex vivo caspase-1 activity in eWAT revealed no difference between the two groups. In contrast, noncanonical inflammasome activation of caspase-11 was significantly augmented in O-HFD than in O-ND; suggesting that membrane pore formation, but not cleavage of pro-IL-1β by caspase-1, is augmented in O-HFD. To examine the membrane pore formation, we performed metabolic activation of bone marrow-derived macrophages (BMDMs). Percentage of pore formation assessed by ethidium bromide staining was significantly higher in BMDMs of O-HFD, accompanied by an enhanced active caspase-11 expression. Consistently, the concentration of IL-1β in culture supernatants was significantly higher in the BMDMs from O-HFD than those from O-ND. CONCLUSIONS These findings demonstrate that maternal HFD exaggerates diet-induced IR in adult offspring by enhancing noncanonical caspase-11-mediated inflammasome activation. Mitochondria are interconnected with lipid droplets, which are of importance in fuel utilization and play a role in muscle insulin sensitivity. Here, we investigated if skeletal muscle lipid content and composition also displays day-night rhythmicity in healthy, lean volunteers. METHODS Skeletal muscle biopsies were obtained from 12 healthy lean male volunteers every 5h over a 24h period. selleck products Volunteers were provided with standardized meals, and biopsies were taken 4.5h after each last meal. CONCLUSIONS Using two distinct methods, our findings show that myocellular lipid content and whole muscle lipid composition varies across the day-night cycle under normal living conditions. In particular, day-night rhythmicity was present in over half of DAG lipid species. Future studies are needed to investigate whether rhythmicity in DAG is functionally related to insulin sensitivity and how this might be altered in prediabetes.
