Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial open data platform and infrastructure that facilitates research on pragmatic trials. It collects and distributes clean trial data, ratings and evaluations using PRECIS-2. This permits a variety of meta-epidemiological studies to compare treatment effect estimates across trials of different levels of pragmatism.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. However, the usage of the term "pragmatic" is inconsistent and its definition and assessment requires further clarification. The purpose of pragmatic trials is to inform clinical practices and policy decisions, not to verify a physiological hypothesis or clinical hypothesis. This difference in primary analysis domain can be explained by the way that most pragmatic trials analyze data. Certain explanatory trials however do not. The overall score was lower for systematic reviews that were pragmatic when the domains on organisation, flexible delivery, and follow-up were merged.
It is crucial to keep in mind that a pragmatic study does not mean that a trial is of poor quality. In fact, there are an increasing number of clinical trials which use the term 'pragmatic' either in their abstracts or titles (as defined by MEDLINE but which is neither sensitive nor precise). These terms may indicate a greater awareness of pragmatism within abstracts and titles, however it's not clear whether this is evident in content.
Conclusions
As the value of real-world evidence becomes increasingly commonplace the pragmatic trial has gained popularity in research. They are randomized trials that evaluate real-world treatment options with new treatments that are being developed. They are conducted with populations of patients closer to those treated in regular care. This approach could help overcome limitations of observational studies that are prone to biases that arise from relying on volunteers, and the limited availability and coding variability in national registries.
Other advantages of pragmatic trials include the ability to utilize existing data sources, and a greater chance of detecting meaningful changes than traditional trials. However, pragmatic trials may be prone to limitations that compromise their credibility and generalizability. For instance the participation rates in certain trials could be lower than expected due to the healthy-volunteer effect and incentives to pay or compete for participants from other research studies (e.g. industry trials). Practical trials are often restricted by the need to recruit participants on time. In addition some pragmatic trials do not have controls to ensure that the observed differences aren't due to biases in the conduct of trials.
The authors of the Pragmatic Free Trial Meta identified RCTs that were published between 2022 and 2022 that self-described themselves as pragmatic. The PRECIS-2 tool was used to evaluate pragmatism.
