Additionally pragmatic trials should strive to make their findings as applicable to real-world clinical practice as possible by making sure that their primary analysis is based on the intention-to-treat method (as described in CONSORT extensions for pragmatic trials).

Many RCTs that do not meet the criteria for pragmatism however, they have characteristics that are contrary to pragmatism, have been published in journals of different kinds and incorrectly labeled pragmatic. This can lead to misleading claims of pragmatism and the use of the term should be made more uniform. The development of a PRECIS-2 tool that provides an objective, standardized evaluation of the pragmatic characteristics is a first step.

Methods

In a pragmatic study the aim is to inform policy or clinical decisions by demonstrating how an intervention can be integrated into routine treatment in real-world contexts. Explanatory trials test hypotheses regarding the cause-effect relationship within idealised settings. In this way, pragmatic trials can have lower internal validity than explanatory studies and be more susceptible to biases in their design analysis, conduct, and design. Despite their limitations, pragmatic research can be a valuable source of information to make decisions in the context of healthcare.

The use of these terms in titles and abstracts could indicate a greater understanding of the importance of pragmatism, however, it is not clear if this is reflected in the content of the articles.

Conclusions

In recent years, pragmatic trials are gaining popularity in research as the value of real-world evidence is becoming increasingly acknowledged. They are clinical trials randomized that evaluate real-world alternatives to care instead of experimental treatments in development. They include patients that are more similar to the ones who are treated in routine care, they use comparators that are used in routine practice (e.g. existing medications) and depend on the self-reporting of participants about outcomes. This approach has the potential to overcome limitations of observational studies, such as the limitations of relying on volunteers, and the limited availability and coding variability in national registry systems.


Pragmatic trials also have advantages, like the ability to leverage existing data sources and a higher probability of detecting meaningful differences than traditional trials. However, they may still have limitations which undermine their effectiveness and generalizability. For instance, participation rates in some trials may be lower than expected due to the healthy-volunteer effect and financial incentives or competition for participants from other research studies (e.g., industry trials). The necessity to recruit people in a timely fashion also limits the sample size and the impact of many pragmatic trials. In addition some pragmatic trials lack controls to ensure that the observed differences are not due to biases in the conduct of trials.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatist and published from 2022. The PRECIS-2 tool was employed to determine the pragmatism of these trials. It covers areas like eligibility criteria and flexibility in recruitment, adherence to intervention, and follow-up. They discovered that 14 of these trials scored pragmatic or highly sensible (i.e., scoring 5 or higher) in one or more of these domains and that the majority were single-center.

Trials with a high pragmatism score tend to have higher eligibility criteria than traditional RCTs which have very specific criteria that are not likely to be used in the clinical setting, and include populations from a wide range of hospitals. The authors claim that these characteristics could make pragmatic trials more meaningful and applicable to everyday practice, but they do not necessarily guarantee that a pragmatic trial is free from bias. The pragmatism principle is not a fixed attribute and a test that does not possess all the characteristics of an explanation study may still yield valuable and valid results.
