Universal severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2; i.e., the causative agent of coronavirus disease 2019 [COVID-19]) polymerase chain reaction (PCR) screening before admission has been adopted by several hospitals to prevent nosocomial SARS-CoV-2 transmission from asymptomatic and pre-symptomatic patients. However, screening usefulness remains unclear because it depends on the regional COVID-19 prevalence, and only a few large-scale studies have been reported. Here we describe the universal PCR screening performed in our hospital before admission of more than 12,000 patients and their attendants to evaluate the usefulness of the screening.
We retrospectively described the universal PCR screening results for asymptomatic patients and their attendants before planned admissions at a hospital in Tokyo, Japan, from August 3, 2020, through March 31, 2021. Nasopharyngeal swab samples were collected at an in-hospital PCR center.
In total, 12,133 persons (11,859 asymptomatic patients and 274 attendants) underwent PCR screening; nine (0.07%) tested positive for SARS-CoV-2 RNA.

Universal PCR screening may be useful for the advanced detection of SARS-CoV-2 infected patients with or without symptoms, which can be a potential source of nosocomial SARS-CoV-2 transmission.Incorporating higher-order optimization functions, such as Levenberg-Marquardt (LM) have revealed better generalizable solutions for deep learning problems. However, these higher-order optimization functions suffer from very large processing time and training complexity especially as training datasets become large, such as in multi-view classification problems, where finding global optima is a very costly problem. To solve this issue, we develop a solution for LM-enabled classification with, to the best of knowledge first-time implementation of hinge loss, for multiview classification. Hinge loss allows the neural network to converge faster and perform better than other loss functions such as logistic or square loss rates. We prove our method by experimenting with various multiclass classification challenges of varying complexity and training data size. The empirical results show the training time and accuracy rates achieved, highlighting how our method outperforms in all cases, especially when training time is limited. Our paper presents important results in the relationship between optimization and loss functions and how these can impact deep learning problems.
Regular physical activity (PA) and healthy body weight have proven benefits on survival in breast cancer (BC) survivors. find more We aimed to define predictors of long-term PA and weight gain in a representative sample of BC survivors.
Data were analysed from 723 women with BC who participated in both the 2012 and 2015 French National VICAN surveys.
Five years after diagnosis, 26.0, 60.6, and 13.4% of BC survivors reported regular, occasional and no PA, respectively. Moreover, 27.4% had a weight gain ≥5kg. In multinomial logistic regressions, regular and occasional PA were both associated with not having depressive disorders, with higher post-traumatic growth, and with a healthy and stable Body Mass Index. Occasional PA was associated with the use of non-conventional medicine, and regular PA with better mental quality of life and normal arm mobility. Weight gain ≥5kg was associated with younger age, heavier body weight at diagnosis, and lymphedema 5 years after diagnosis.
Mental well-being is associated with successful long-term patient investment in PA. Psychological support and early management of disease sequelae are needed to help ensure BC survivors engage in and maintain healthy lifestyles.

In the present study, we assessed the gene mutations and copy number alterations that recurrently occurred in cetuximab-treated patients with metastatic colorectal cancer (mCRC). Targeted next-generation sequencing was performed in the tumor samples obtained pre- and postcetuximab treatment to assess the variations that occurred during cetuximab treatment. Moreover, we identified the emergent gene mutations (CDK6, EPHA3, ERCC2, MYC, PCMTD1, PIK3CA, PRIM2, RICTOR, and ZNRF3) and copy number alterations (ARAF, BCL2, BRCA2, EGFR, MYC, and SMAD4) that were recurrently observed only in postprogression samples and not in pretreatment or posttreatment samples from patients revealing clinical response. Furthermore, to identify the feasible candidate variations implicated in treatment resistance, we examined the variants with clonal expansion during treatment and discovered PCBP1 as a variant associated with posttreatment progression. Various recurrent mutations were enriched in the TGF-beta signaling pathway. Collectively, we identified recurrent variations in mCRC samples exhibiting post-cetuximab progression. Online sexual harassment in adolescence is associated with depressive symptoms. There is, however, a dearth of research investigating variability of symptom profiles in this population in relation to offender gender and age.
To identify the proportion of adolescents reporting online harassment by different types of offenders and compare their levels of depression.
Participants were 18,872 Chilean students aged 12 to 17years (3.063 of them online sexually harassed).
The study involved a secondary analysis of self-report data on online sexual harassment, poly-victimization, and depression collected as part of the National Poly-victimization Survey.
In 37.6% of the cases the offender was male under 18, in 22.4% an adult male, in 14.5% a female under 18, and in 2.9% an adult female. In 22.5% of cases the offender could not be identified. An ANCOVA demonstrated levels of poly-victimization across the lifespan and frequency of online sexual harassment in the last year to predict depressive symptomatology. In females, higher levels of depressive symptoms were observed among those sexually harassed by either a female under 18, an offender whose age and gender the victim could not identify, or an adult male. In males, higher levels of depression were observed among those harassed by either an adult male, an offender whose age and gender the victim could not identify, or a male under 18.
The current study highlights the importance of offender's age and gender in predicting depression levels in adolescent victims of online sexual harassment.

Chicken anaemia virus (CAV) and infectious bursal disease virus (IBDV) are common causes of immunosuppression in chickens. Immunosuppression was induced by experimental infection with either CAV or IBDV to assess the effect of immunosuppression on the efficacy of vaccination with Mycoplasma gallisepticum strain ts-304 against infection with virulent M. gallisepticum, a common bacterial pathogen of chickens worldwide. Birds were experimentally infected with either CAV or IBDV at 1 week of age, before vaccination and challenge with M. gallisepticum to examine the effect of immunosuppression at the time of vaccination, or at 6 weeks of age, after vaccination against M. gallisepticum but before challenge with virulent M. gallisepticum, to investigate the effect of immunosuppression at the time of challenge. All birds were vaccinated with a single dose of the ts-304 vaccine at 3 weeks of age and experimentally challenged with the virulent M. gallisepticum strain Ap3AS at 8 weeks of age. In immunosuppressed chickens there was a reduction in protection offered by the ts-304 vaccine at two weeks after challenge, as measured by tracheal mucosal thicknesses, serum antibody levels against M. gallisepticum, air sac lesion scores and virulent M. gallisepticum load in the trachea. Immunosuppressed birds with detectable serum antibodies against M. gallisepticum were less likely to have tracheal lesions. This study has shown that immunosuppression caused by infection with CAV or IBDV can interfere with vaccination against mycoplasmosis in chickens.
The field of online monitoring of the beam range is one of the most researched topics in proton therapy over the last decade. The development of detectors that can be used for beam range verification under clinical conditions is a challenging task. One promising possible solution are modalities that record prompt-gamma radiation produced by the interactions of the proton beam with the target tissue. A good understanding of the energy spectra of the prompt gammas and the yields in certain energy regions is crucial for a successful design of a prompt-gamma detector. Monte-Carlo simulations are an important tool in development and testing of detector concepts, thus the proper modelling of the prompt-gamma emission in those simulations are of vital importance. In this paper, we confront a number of GEANT4 simulations of prompt-gamma emission, performed with different versions of the package and different physics lists, with experimental data obtained from a phantom irradiation with proton beams of four different energies in the range 70-230 MeV.
The comparison is made on different levels features of the prompt-gamma energy spectrum, gamma emission depth profiles for discrete transitions and the width of the distal fall-off in those profiles.
The best agreement between the measurements and the simulations is found for the GEANT4 version 10.4.2 and the reference physics list QGSP_BIC_HP.
