American Academy of Audiology.BACKGROUND Pediatric hearing instrument fitting is optimally performed with individually obtained real-ear-to-coupler difference (RECD) measurements. If these measurements cannot be obtained, predicted values based on age are used. Recent evidence obtained from children aged 3-11 years suggests that head circumference (HC) may be a viable alternative or addition to age for use in RECD prediction. PURPOSE The purpose of the present study was to determine if HC can be used to predict RECDs in infants, children, and adults. RESEARCH DESIGN A correlational design was used. HC and RECD values were measured in all participants. STUDY SAMPLE Participants were 278 North American infants and children (136 males and 142 females) aged 1.6 months to 11 years and 109 adults (42 males and 67 females) aged 18 years to 83 years. DATA COLLECTION AND ANALYSIS After otoscopic inspection and immittance measurements were performed to assess candidacy for inclusion in the study, HC was measured twice for all participants and a single RECD measure was obtained for each participant at twelve frequencies (250 through 12500 Hz). The reliability of HC measurements was assessed with an intraclass correlation analysis. Linear regression analyses were performed with age and HC as predictor variables and RECDs as the dependent variable. RESULTS Analysis indicated good reliability of the HC measurement. The relationships between RECD and HC were comparable with the relationships between RECD and age. Combining HC and age did not improve predictive accuracy. CONCLUSIONS HC can be used in children and adults as an alternative metric in the prediction of RECDs when individual RECDs cannot be obtained. American Academy of Audiology.BACKGROUND Previous studies in a large population of typically developing (TD) children and a small clinical group showed high correlations between the dichotic and diotic conditions of the Dichotic Digits difference Test (DDdT), as well as between DDdT performance and measures of memory and attention. PURPOSE The purpose of the study was to investigate the performance on the DDdT in a large clinical sample. RESEARCH DESIGN Correlational analysis between the DDdT diotic condition and the dichotic free recall (FR) right-ear, left-ear, and total (ear-averaged) conditions, as well as between DDdT and memory performance. STUDY SAMPLE One hundred one children (6 years 3 months to 15 years 0 month, mean 9 years 6 months) were referred for assessment to the Australian Hearing Central Auditory Processing Disorder (CAPD) service. Results were compared with data from 112 TD children collected from previously published studies. DATA COLLECTION AND ANALYSIS Z-scores were used to account for the effect of age on performan dichotic performance. Random measurement error accounted for a further 16%. CONCLUSIONS Factors other than dichotic performance strongly impact a child's ability to perform a dichotic digit listening task. This result has widespread implications in respect to the interpretation of CAPD test results. American Academy of Audiology.BACKGROUND Despite improvements in cochlear implant (CI) technology, pediatric CI recipients continue to have more difficulty understanding speech than their typically hearing peers in background noise. Bafilomycin A1 A variety of strategies have been evaluated to help mitigate this disparity, such as signal processing, remote microphone technology, and microphone placement. Previous studies regarding microphone placement used speech processors that are now dated, and most studies investigating the improvement of speech recognition in background noise included adult listeners only. PURPOSE The purpose of the present study was to investigate the effects of microphone location and beamforming technology on speech understanding for pediatric CI recipients in noise. RESEARCH DESIGN A prospective, repeated-measures, within-participant design was used to compare performance across listening conditions. STUDY SAMPLE A total of nine children (aged 6.6 to 15.3 years) with at least one Advanced Bionics CI were recruited for this stud, suggesting that a child's listening performance in noise will not be compromised by choosing the microphone configuration best suited for the child. UltraZoom (adaptive beamformer) yielded higher performance than all omnidirectional microphones in moderate background noise for adolescents aged 9 to 15 years. The implications of these data suggest that for older children who are able to reliably use manual controls, UltraZoom will yield significantly higher performance in background noise when the target is in front of the listener. American Academy of Audiology.BACKGROUND Acceptance of background noise serves as a means to predict likelihood of hearing aid success. Individuals that are able to accept background noise are more likely to be successful with hearing aids. PURPOSE The aim of the study was to assess the impact of sound enrichment on the acceptable noise level (ANL). STUDY SAMPLE Nineteen young adult participants served as listeners. Participants were randomly assigned to the experimental or control group. RESEARCH DESIGN An experimental design with random assignment to experimental or control group was used. DATA COLLECTION AND ANALYSIS One group used sound enrichment procedures for 2 weeks, whereas the other group served as a control group. Sound enrichment procedures required that participants add low-level background sound to any quiet environment encountered during the study. Most comfortable listening level (MCL) and background noise level (BNL) were measured at three sessions, each 1 week apart (baseline, after 1 week of treatment, and after 2 weeks of treatment). RESULTS Analytical statistics revealed that ANL improved for the sound enrichment group but remained the same for the control group. For both groups, there was no significant change in MCL across sessions. However, for the experimental group, BNL improved (increased) over the 2-week period while using sound enrichment. CONCLUSIONS Results of this study indicate that ANL can be improved with the use of sound enrichment procedures over a 2-week period. Future work should examine the use of sound enrichment procedures for older adults with hearing loss. American Academy of Audiology.In the past two decades, major advances in biomedical intervention approaches to prevent HIV and many sexually transmissible infections (STIs) have shown great promise. However, challenges to prevention remain in the area of achieving population-level impact for biomedical prevention approaches. In this paper we address what social and behavioural research approaches can contribute beyond well-known behaviour change and counselling interventions. We organise work into five areas. Adherence and disinhibition research is primarily into individual-level constructs pertaining to maximising intervention effectiveness. Coverage research represents a population-level construct germane to maximising efficient prioritisation for prevention. Research covering social determinants, a second population-level construct, contributes to both prioritisation and effectiveness. Finally, disparities and social inequities need to be incorporated into prevention, given the pervasive and persistent disparities found in rates of HIV and STIs and in their antecedents.BackgroundGay and other men who have sex with men of Asian background (GAM) have been identified as a key population in efforts to eradicate HIV in New South Wales. The aims of the present study were to evaluate current levels of engagement with HIV and sexually transmissible infection (STI) testing services, assess knowledge of pre- and post-exposure prophylaxis and to identify factors associated with service engagement in this group. Methods A survey of 604 GAM residing in Sydney and Melbourne was undertaken. Results The data identified that a significant proportion of non-HIV-positive men (i.e. HIV-negative men and men whose HIV status was unknown) surveyed (n = 567; 93.9%) had engaged in frequent HIV testing and comprehensive STI testing in the 12 months prior to the survey (n = 180; 31.7%). There were significant differences (P less then 0.05) in sexual practices at the bivariate level between those who reported frequent and comprehensive HIV/STI testing and those who did not. Those who tested regularlidentify as being at high HIV and STI risk will require the use of novel routes by which to disseminate this information.Piperacillin/tazobactam is a commonly prescribed antimicrobial agent. Tenofovir alafenamide (TAF) is increasingly being used in antiretroviral therapy (ART) of HIV. Herein we report a case of a 57-year-old male with AIDS receiving TAF-containing ART in whom severe refractory hypokalaemia developed after coadministration of piperacillin/tazobactam for suspected hospital-acquired infection. Upon withdrawal of piperacillin/tazobactam, serum potassium concentrations returned to normal within 2 days. Hypokalaemia is a rare adverse effect of piperacillin/tazobactam and may be aggravated with the underlying use of TAF. We also reviewed past reported cases of hypokalaemia after piperacillin/tazobactam administration. We want to highlight that a more cautious approach should be considered when combining piperacillin/tazobactam and TAF in clinical practice.Aging can cause retinal degeneration, which leads to visual impairment among the elderly population. Age-dependent increases in amyloid beta (Aβ) inducesinflammatory cytokine overexpression in the retinal pigment epithelium (RPE), which promotes the progression of age-related retinal degeneration. However, whether dietary antioxidants are useful for the treatment of RPE degeneration remains to be clarified. This study exposited the protective activities and underlying mechanisms of grape seed extracts (GSEs) against Aβ-induced proinflammatory events in mouse retinas and ARPE-19 cells. The experimental data demonstrated that GSEs attenuated the increases in messenger RNA (mRNA) levels of interleukin 12 (IL-12), interleukin 6 (IL-6), interleukin 1β (IL-1β), and interleukin 18 (IL-18) in the retinal tissues of Aβ-treated mice. The experimental results in mice were confirmed by findings in ARPE-19 cells with or without treatment with GSEs. GSEs affected the protein expression levels of endoplasmic reticulum stress markers in ARPE-19 cells exposed to Aβ. Knockdown of Bip blocked the inhibitory activities of GSEs on mRNA levels of IL-6, IL-1β, IL-18, and IL-8. We conclude that GSEs may suppress proinflammatory cytokines partly by increasing the expression of Bip.A variety of natural compounds have been used to reduce the oxidative stress associated with Alzheimer's disease, and many of these defend cells from oxidative stress-induced neuronal toxicity. In this study, the protective effect of radish (Raphanus raphanistrum) extract was investigated in mice and PC12 cells. In vivo behavioral tests were completed to examine the protective effects of the extract on amyloid beta (Aβ)-peptide1-42-induced learning and memory impairment in a mouse model. The extract increased spontaneous alternation behaviors and step-through latency in mice. We discovered that administration of the extract reduced lipid peroxidation and Aβ aggregation in a biochemical study of mice brain tissues. Treatment with the extract also increased acetylcholine and catalase activity in the brain. Furthermore, the radish extract attenuated H2O2-induced oxidative stress in cells. Through sequential fractionation of the radish extract, the active compound was identified as oleamide. These results suggest that the radish extract could have a protective role against oxidative stress-induced neuronal toxicity, possibly owing to the antioxidative activity of oleamide.
