Clinical trials of the messenger ribonucleic acid (mRNA)-1273 vaccine developed by Moderna proved excellent safety and efficacy for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) prevention. However, the Centers for Disease Control and Prevention (CDC) has been investigating cases of myocarditis and pericarditis reported in the Vaccine Adverse Event Reporting System (VAERS) database. Currently, the CDC is reporting rates of 40.6 cases per million after second doses of mRNA vaccines administered to males 30 years or younger. Notably, the initial vaccine trials consisted of a limited number of adolescents and young adults; therefore, they were likely not powered to detect this rare potential side effect. We present a case of transient myopericarditis occurring in a young and healthy patient within 48 h of his second vaccination dose. Although a definitive causal relationship has yet to be determined, we came to this correlation because of the temporal association seen in our patient, secondary to the second dose of vaccination. Furthermore, we also suspect an autoimmune mechanism as the cause of cardiac injury, augmented by the increased vaccine reactogenicity seen in younger patients.Pregnancy and lactation-associated osteoporosis (PLO) is a disease caused by vertebral compression fracture, and it is characterized by low back pain during pregnancy or the postpartum period. However, it is difficult to predict and prevent PLO prepartum in high-risk groups. Recently, long-term tocolysis with magnesium sulfate (MgSO4) has been reported to be associated with PLO. The purpose of this case series was to assess postpartum bone mass after long-term tocolysis with MgSO4 and accumulated doses of MgSO4. We report the case of a pregnant woman with vertebral compression fractures during pregnancy following long-term tocolysis with MgSO4. We investigated whether long-term tocolysis with MgSO4 was a high risk factor for PLO. Therefore, we retrospectively evaluated bone mineral density after delivery in nine women who had long-term tocolysis with MgSO4 (more than 8 days) for treatment of threatened preterm birth at our hospital from January 2020 to December 2020. The age of the women was between 20 and 41 years (mean age, 30 years). The body mass index of the women was between 18.1 and 25.4 kg/m2 (mean 20.0 kg/m2). Three women had a positive smoking history, and none had a family history of osteoporosis. The average duration of tocolysis with MgSO4 was 11 - 97 days. The accumulated doses of MgSO4 were between 168 and 3,756 g. Four of nine cases were diagnosed with low bone mass of young adult mean (YAM) value ≤ 80%. Of them, one case (accumulated doses of MgSO4 1,260 g) was diagnosed with PLO of YAM value ≤ 70%, and one case (accumulated doses of MgSO4 3,756 g) was diagnosed with bone fracture with a YAM value of ≤ 70%. Long-term tocolysis with MgSO4 may be suggested as one of the risk factors of PLO. Nutritional guidance and rehabilitation are important interventions for target patients.Systemic lupus erythematosus (SLE) is a chronic autoimmune disease that affects multiple organs. Infectious agents have been implicated in the pathogenesis of SLE. The emergent severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) induces a pro-inflammatory cytokine storm and has been linked to autoimmune phenomena, which can lead to the onset of autoimmune diseases. We report the case of a 70-year-old patient who developed a toxic epidermal necrolysis (TEN)-like subacute cutaneous lupus (SCL) as a severe presentation of SLE, 1 month after SARS-CoV-2 infection. After excluding other causes of SLE, treatment was initiated with a successful outcome.Glutaric aciduria type 2 is a rare inborn disease of fatty acid metabolism. The clinical manifestation of this disease is heterogeneous and ranges from severe neonatal forms to mild late-onset forms. We present a case of a previously healthy 22-year-old woman with unexplainable hypoglycemia and encephalopathic hyperammonemia. Acylcarnitine profile and organic acids analysis were compatible with glutaric aciduria type 2. On suspicion of this disease, the patient started supplements with carnitine and riboflavin, along with hemodialysis with a complete recovery. The genetic test confirmed the diagnosis. Glutaric aciduria type 2 has no cure and the metabolic decompensation can be a severe event, but treatable and preventable, if this pathology gets recognized.Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection has emerged as a public health hazard. It is commonly associated with respiratory symptoms including fever, cough and sore throat. Viral infections have historically been implicated in the exacerbations of gastrointestinal (GI) disorders. Coronavirus disease 2019 (COVID-19) has been associated with GI manifestations including diarrhea and vomiting. However, exacerbation of underlying functional GI disorders has been rarely reported with SARS-CoV-2 infection. Herein we report a case of a 31-year-old female with acute gastroparesis flare secondary to COVID-19 which was refractory to pharmacological therapy.A 45-year-old patient with spina bifida and adenocarcinoma of the rectum was treated with a superior hypogastric plexus (SHP) ablation for pain control. The procedure enabled her to reduce opioid consumption, being more clear-headed and functional to be discharged to her residence. The case is presented to highlight the options of neurolytic interventions to manage pain in terminally ill cancer patients. We discuss the options of SHP ablation and justify our choice of approach and the use of a neurolytic agent.Mycobacterium tuberculosis is primarily known to affect the lungs with cavitary lesions and enlarged lymph nodes as the first telltale sign. However, if the bacteria spread to extrapulmonary areas such as the bones, and lack lymphadenopathy, then the differential diagnosis may become misleading. We present a case of a 68-year-old male patient with a chief complaint of chronic left hip pain upon which computer tomography identified lytic lesions on the left hip. Given the mildly elevated prostate-specific antigen with a family history of prostate cancer, a bone biopsy was warranted. The biopsy revealed non-caseating granulomas and the DNA probe identified the Mycobacterium tuberculosis complex. selleck products This case signifies that atypical presentations of Mycobacterium tuberculosis may mimic other diagnoses and more invasive techniques such as a biopsy may be necessary.Radiofrequency catheter ablation (RFA) is an effective treatment for recurrent symptomatic atrial fibrillation (AF) despite medical therapy. Complications include cardiac tamponade, pulmonary vein stenosis, and rarely atrial-esophageal fistula (AEF). We present a case of a 77-year-old male patient with a history of persistent AF status post posterior left atrial isolation 50 days prior, who presented to the emergency department with bilateral lower extremity weakness ongoing for an hour. In addition, he reported a non-productive cough, chest discomfort, and dysphagia. He was hypertensive, febrile, tachycardic, and with tachypnea on initial assessment. Initial labs revealed leukocytosis with elevated troponin level. Brain imaging confirmed subacute infarcts suggestive of embolic disease. Tissue plasminogen activator was contraindicated. He was admitted for management of altered mental status and sepsis of an unknown origin. Due to associated chest discomfort and dysphagia, computed tomography (CT) scan of the a consensus agreement for palliative measures. To our knowledge, this case represents one of the few cases documenting life-threatening infective endocarditis with septic emboli as a complication of AEF from RFA. Therefore, clinicians should have a high index of suspicion due to associated grave prognosis.Mycobacterium szulgai (MS) is a rare and slow-growing type of non-tuberculous mycobacteria (NTM), with a human isolation prevalence of less than 0.2% of all NTM cases. MS may cause pulmonary infection, extra-pulmonary localized disease involving the skin, lymph nodes, bone, synovial tissue or kidneys and disseminated infection, when two or more organs are affected. When disseminated infection is present, the patients usually have an underlying immunosuppressive condition. The authors report the case of a 25-year-old patient with systemic lupus erythematosus, presenting with recurrent fever, non-productive coughing, weight loss and asthenia, as well as two violaceous plaques with superficial ulceration in the gluteal region. MS was isolated from the bronchial lavage and skin biopsy cultures, confirming the rare disseminated form of MS infection. After 10 months of follow-up on isoniazid, rifampin, ethambutol and pyrazinamide, no signs of relapse were evident. To date, only 16 other cases of MS disseminated disease have been reported. The global market for seafood alternatives is witnessing an exponential growth. Nevertheless, the nutritional quality of such products is scarcely studied. Thus, this study aimed to evaluate, for the first time, the nutritional quality of seafood alternatives launched in the global market from 2002 to 2021 and to compare them with the conventional seafood products. Using the Mintel Global New Products Database, the nutritional information of seafood alternatives (i.e., tuna, shrimps, calamari, fish fingers, fish sticks, salmon, caviar, and fillet) was retrieved, and compared with conventional products. A total of 149 seafood alternatives were identified, of which 83 items had complete mandatory nutritional labeling. Conventional products (n = 973) were also collected, from which 130 products have a complete nutritional labeling. Results revealed that tuna, shrimps, caviar and fillet alternatives contained significantly less protein than conventional products, while calamari, fish fingers, fish sticks and salmon alternatives had similar amounts to their conventional counterparts. Salt content was significantly higher in tuna, fish fingers and sticks substitutes, but lower in shrimps, calamari and caviar alternatives compared to conventional products. Overall, the commercially available seafood alternatives have nutritional strengths and some shortcomings to be further addressed in future research such as low protein content. Additionally, fortification of seafood alternatives with micronutrients, such as omega-3 fatty acids and vitamins (A, B, and D), should be considered to ensure a nutritional equivalence with the conventional products.

Heart failure (HF) is a prevalent, serious chronic illness that affects 6.5million adults in the United States. Among patients with HF, the prevalence of attention impairment is reported to range from 15% to 27%. Although attention is fundamental to human activities including HF self-care, cognitive interventions for patients with HF that target improvement in attention are scarce. The COgnitive intervention to Restore attention using nature Environment (CORE) study aims to test the preliminary efficacy of the newly developed Nature-VR, a virtual reality-based cognitive intervention that is based on the restorative effects of nature. Nature-VR development was guided by Attention Restoration Theory. The target outcomes are attention, HF self-care, and health-related quality of life (HRQoL). Our exploratory aims examine the associations between attention and several putative/established HF biomarkers (eg, oxygen saturation, brain-derived neurotrophic factor, apolipoprotein E, dopamine receptor, and dopamine transporter genes) as well as the effect of Nature-VR on cognitive performance in other domains (ie, global cognition, memory, visuospatial, executive function, and language), cardiac and neurological events, and mortality.
