Magnetic resonance-guided focused ultrasound (MRgFUS) is a completely non-invasive technology that has been approved by FDA to treat several diseases. This report, prepared by the American Association of Physicist in Medicine (AAPM) Task Group 241, provides background on MRgFUS technology with a focus on clinical body MRgFUS systems. The report addresses the issues of interest to the medical physics community, specific to the body MRgFUS system configuration, and provides recommendations on how to successfully implement and maintain a clinical MRgFUS program. The following sections describe the key features of typical MRgFUS systems and clinical workflow and provide key points and best practices for the medical physicist. Commonly used terms, metrics and physics are defined and sources of uncertainty that affect MRgFUS procedures are described. Finally, safety and quality assurance procedures are explained, the recommended role of the medical physicist in MRgFUS procedures is described, and regulatory requirements for planning clinical trials are detailed. Although this report is limited in scope to clinical body MRgFUS systems that are approved or currently undergoing clinical trials in the United States, much of the material presented is also applicable to systems designed for other applications.This essay considers the evolution of the author's research over the last 15 years in which the treatment of castration as a historical problem holds promise for bridging disparate scholarly fields and paradigms. In particular, by tracing the shift in the author's intellectual focus from the science of sex change to the history of transcultural psychoanalysis, some insights are offered in regard to the intertwined politics of transness, Chineseness, and the unconscious. Though psychoanalysis may appear as a subject far removed from the eunuchs of ancient China, this essay highlights some of the methodological stakes that have saturated the historical study of both topics. These reflections can serve as a touchstone for thinking beyond disciplinary norms and conventions, especially in Chinese studies and the history of science.
Accurate segmentation of complex tumors in lung computed tomography (CT) images is essential to improve the effectiveness and safety of lung cancer treatment. However, the characteristics of heterogeneity, blurred boundaries, and large-area adhesion to tissues with similar gray-scale features always make the segmentation of complex tumors difficult.
This study proposes an effective deep network for the automatic segmentation of complex lung tumors (CLT-Net). The network architecture uses an encoder-decoder model that combines long and short skip connections and a global attention unit to identify target regions using multiscale semantic information. A boundary-aware loss function integrating Tversky loss and boundary loss based on the level-set calculation is designed to improve the network's ability to perceive boundary positions of difficult-to-segment (DTS) tumors. We use a dynamic weighting strategy to balance the contributions of the two parts of the loss function.
The proposed method was verified rrent mainstream lung tumor segmentation networks in terms of Dice similarity coefficient and Hausdorff distance. Note that the proposed method is not limited to the segmentation of complex lung tumors but also performs in different modalities of medical image segmentation.
In previous work, we demonstrated that mixed electron-photon radiation therapy (MBRT) produces treatment plans with improved normal tissue sparing and similar target coverage, when compared to photon-only plans. The purpose of this work was to validate the MBRT delivery process on a Varian TrueBeam accelerator and laying the groundwork for a patient-specific quality assurance (QA) protocol based on ion chamber point measurements and 2D film measurements.
MC beam models used to calculate the MBRT dose distributions of each modality (photons/electrons) were validated with a single-angle beam MBRT treatment plan delivered on a slab of Solid Water phantom with a film positioned at a depth of 2cm. The measured film absorbed dose was compared to the calculated dose. However, the detection of straightforward effects has been proven difficult due to the complexity and diversity of reproductive landscapes found in different taxa. Here, we compare the possible effect of relative testes mass as a sperm competition proxy on protamine genotype (protamine 1/protamine 2 ratio) and the link to sperm head phenotype in two rodent groups, mice, and voles. In mice, protamine expression ratios were found to increase from low values toward a 11 ratio in a positive association with testes mass, and relative sperm head area. In contrast, in voles, decreasing protamine expression ratios were found in species with larger testes but, surprisingly, they range from high values, again toward a 11 ratio, and showing a negative correlation with relative sperm head area. Altogether, we found differences in the way protamines seem to be selected and involved in adaptations of the sperm head in voles and mice. However, sexual selection driven by sperm competition seems to exhibit a common evolutionary pattern in both groups toward an equilibrium in the expression of the two protamines.
Ischemic stroke, as well as intracerebral hemorrhage (ICH), involving the insular cortex tends to be more severe. The impact of insular involvement on outcome of ICH remains enigmatic.
We analyzed 159 patients with supratentorial ICH. Depending on insular involvement the patients were classified into two groups (ICH

The main finding was an association of insular involvement of ICH with worse short-term outcome as measured by mRS (common odds ratio 4.08 (95% CI 2.09-7.92); p<.001). This association survived adjustment to relevant covariates such as age, sex, ICH volume, intraventricular hemorrhage, pneumonia, and length of stay (adjusted common odds ratio 2.51 (95% CI 1.21-5.21); p=.014) but had no predictive value for side of ICH or rate of atrial fibrillation. There was no association of ICH localization with in-hospital death rate.

A lot of what we know about γδT17 cells stems from mouse models, however, increasing evidence implicates these cells in numerous human diseases. Herein, we aim to give an overview of the most common mouse models that have been used to study the role of γδT17 cells in infection, inflammation, and cancer, while at the same time we will evaluate evidence for their importance in humans. We hope and believe that in the next 10 years, means to take advantage of the protective and destructive properties of γδ T and in particular γδT17 cells will be part of our standard immunotherapy toolkit.
Caesarean section (CS) is associated with an increased risk of adverse health outcomes for both mothers and offspring. The evidence for an association between CS and reduced offspring cognitive and academic performance has been inconsistent, with considerable limitations.
The aim of this study is to compare cognitive and academic performance in childhood and early adulthood in offspring delivered by CS with those delivered vaginally at term.
Data on 4327 mothers and offspring from a longitudinal birth cohort study were analysed. Offspring cognitive performance was measured by the Picture Peabody Vocabulary Test-Revised (PPVT-R) at ages five and 21 and the Raven's Standard Progressive Matrices at age 14. Academic achievement was assessed using the Wide Range Achievement Test at age 14.
After adjustment for confounding factors, there was no statistically significant association between cognitive performance and offspring birth mode at age five (P=0.11). The adjusted difference of mean scores at five years on the PPVT-R for elective CS birth compared to those born by vaginal delivery was -2.2 (95% confidence interval (CI) -4.3 to -0.2), whereas for emergency CS it was 0.0 (95% CI -2.0 to 2.0). There were no differences in cognitive or academic performance at ages 14 and 21.
Birth mode was not significantly associated with offspring cognitive or academic performance. Our study does not support concerns that CS is associated with a reduction in cognitive performance.
