Methods

In a pragmatic study the aim is to inform clinical or policy decisions by showing how an intervention could be integrated into routine treatment in real-world situations. Explanatory trials test hypotheses about the causal-effect relationship in idealized environments. In this way, pragmatic trials can have less internal validity than explanation studies and be more prone to biases in their design as well as analysis and conduct. Despite these limitations, pragmatic trials may contribute valuable information to decision-making in the context of healthcare.

The framework consisted of nine domains evaluated on a scale of 1-5 with 1 being more explanatory while 5 was more pragmatic. These terms may signal an increased awareness of pragmatism within titles and abstracts, but it's not clear whether this is evident in the content.

Conclusions

As the importance of real-world evidence becomes increasingly widespread, pragmatic trials have gained traction in research. They are randomized clinical trials which compare real-world treatment options instead of experimental treatments under development, they involve patient populations that are more similar to the patients who receive routine care, they use comparators which exist in routine practice (e.g., existing drugs), and they rely on participant self-report of outcomes. This approach has the potential to overcome the limitations of observational studies that are prone to biases associated with reliance on volunteers and the lack of availability and coding variability in national registry systems.

Pragmatic trials offer other advantages, including the ability to leverage existing data sources and a higher likelihood of detecting meaningful differences from traditional trials. However, pragmatic trials may be prone to limitations that compromise their credibility and generalizability. For instance the rates of participation in some trials could be lower than anticipated due to the healthy-volunteer effect and incentives to pay or compete for participants from other research studies (e.g., industry trials). Many pragmatic trials are also restricted by the need to recruit participants quickly. Certain pragmatic trials lack controls to ensure that the observed differences aren't caused by biases in the trial.

The authors of the Pragmatic Free Trial Meta identified 48 RCTs self-labeled as pragmatist and published until 2022. The PRECIS-2 tool was used to evaluate the degree of pragmatism. It covers areas such as eligibility criteria and flexibility in recruitment as well as adherence to interventions and follow-up. They found 14 trials scored highly pragmatic or pragmatic (i.e. scoring 5 or above) in at least one of these domains.
