We propose two novel tuning parameters for estimating the ROC curve via Bernstein polynomial smoothing of the empirical ROC curve. The new estimator is very easy to implement with the naturally selected tuning parameter, as illustrated by analyzing both real and simulated data sets. Empirical performance is investigated through extensive simulation studies with a variety of scenarios where the two groups are both from a single family of distributions (symmetric or right skewed) or one from a symmetric and the other from a right skewed distribution. The new estimator is uniformly more efficient than the empirical ROC estimator, and very competitive to eleven other existing smooth ROC estimators in terms of mean integrated square errors.
The novel coronavirus pandemic has affected emergency department consultations for surgical pathologies. The aim of our study was to compare the number of acute appendicitis cases and the proportion of complicated appendicitis before and during the COVID-19 pandemic.

We retrospectively analyzed all data collected from a multi-center database of patients presenting to the emergency department for acute appendicitis during the COVID-19 pandemic from March 12 to June 6, 2020, and compared these data with those from the same periods in 2017, 2018, and 2019. The number of acute appendicitis cases, proportion of complicated appendicitis, and pre- and postoperative patient characteristics were evaluated.

A total of 306 patients were included in this evaluation. Sixty-five patients presented during the 2020 COVID-19 pandemic lockdown (group A), and 241 patients in previous years (group B 2017-2019). The number of consultations for acute appendicitis decreased by almost 20 percent during the pandemic compared wi likely due to patient delay in consulting the emergency department at symptom onset. Patients and general practitioners should be aware of this problem to avoid a time delay from initial symptoms to consultation.
The effects of aging on physical and mental health may be ameliorated by regular participation in physical activity (PA). There is also evidence for the benefits of various training modalities on cognition and functional ability in older adults. The aim of this study was to compare effects of a 12-week active video gaming intervention (X Box Kinect Sports) to conventional multimodal supervised exercise on fitness, functional ability and cognitive performance in older adults with memory complaints.

Participants (n = 45, 72±5 yrs.) were recruited from 6 retirement homes and cluster-randomized into the Interactive Video Gaming (IVG) group (N = 23) or Conventional Multimodal (CM) group (N = 22), meeting 2 x 1 hour sessions, weekly for 12 weeks. Pre-post measures included 6 min walk, timed up and go, dynamic balance, functional reach, Mini-Mental State Examination, N-back Task and the Modified Stroop task.

The IVG group demonstrated significant improvement in the total number correct responses on the Stroop task (P = 0.028) and for average reaction time of correct colour-words (P = 0.024), compared to the CM group. Functional ability improved significantly in the IVG group, including the 6-min walk (P = 0.017), dynamic balance (P = 0.03), timed up and go (P<0.001) and functional reach (P<0.0010).

An active interactive video gaming intervention was more effective than conventional multimodal exercise in improving executive and global cognitive performance and functional capacity in older adults with subjective memory complaints.

Pan African Clinical Trial Registry-PACTR202008547335106.Glaucoma is a leading cause of blindness worldwide whose detection is based on multiple factors, including measuring the cup to disc ratio, retinal nerve fiber layer and visual field defects. Advances in image processing and machine learning have allowed the development of automated approached for segmenting objects from fundus images. However, to build a robust system, a reliable ground truth dataset is required for proper training and validation of the model. In this study, we investigate the level of agreement in properly detecting the retinal disc in fundus images using an online portal built for such purposes. Two Doctors of Optometry independently traced the discs for 159 fundus images obtained from publicly available datasets using a purpose-built online portal. Additionally, we studied the effectiveness of ellipse fitting in handling misalignments in tracing. We measured tracing precision, interobserver variability, and average boundary distance between the results provided by ophthalmologists, and optometrist tracing. We also studied whether ellipse fitting has a positive or negative impact on properly detecting disc boundaries. The overall agreement between the optometrists in terms of locating the disc region in these images was 0.87. However, we found that there was a fair agreement on the disc border with kappa = 0.21. Disagreements were mainly in fundus images obtained from glaucomatous patients. The resulting dataset was deemed to be an acceptable ground truth dataset for training a validation of models for automatic detection of objects in fundus images.To explore the possible emergence and lived consequences of social inequality in the Atacama, we analyzed a large set (n = 288) of incredibly well preserved and contextualized human skeletons from the broad Middle Period (AD 500-1000) of the San Pedro de Atacama (Chile) oases. In this work, we explore model-based paleodietary reconstruction of the results of stable isotope analysis of human bone collagen and hydroxyapatite. The results of this modeling are used to explore local phenomena, the nature of the Middle Period, and the interaction between local situations and the larger world in which the oases were enmeshed by identifying the temporal, spatial, and biocultural correlates and dimensions of dietary difference. Our analyses revealed that 1) over the 600-year period represented by our sample, there were significant changes in consumption patterns that may evince broad diachronic changes in the structure of Atacameño society, and 2) at/near 600 calAD, there was a possible episode of social discontinuity that manifested in significant changes in consumption practices. Additionally, while there were some differences in the level of internal dietary variability among the ayllus, once time was fully considered, none of the ayllus stood out for having a more (or less) clearly internally differentiated cuisine. Finally, sex does not appear to have been a particularly salient driver of observed dietary differences here. While we do not see any de facto evidence for complete dietary differentiation (as there is always overlap in consumption among individuals, ayllus, and time periods, and as isotopic analysis is not capable of pinpointing different foods items or preparations), there are broad aspects of dietary composition changing over time that are potentially linked to status, and foreignness. Standard laparoscopic right-sided hemicolectomy with D-3 lymph node dissection was accompanied by resection of a conglomerate of nodal metastases behind the pancreatic head and superficial resection of the pancreas. Extra-regional lymph node excision is a reasonable option for colon mucinous adenocarcinoma stage I-III. However, comprehensive preoperative examination is required. Technical difficulty of extra-regional lymph node excision it is not the reason for limitation of surgical intervention. This review is devoted to the most common methods of contrast-enhanced MRI for assessment of liver function and correlation between severity of diffuse structural liver changes and gadoxetic acid accumulation in liver parenchyma. There is no a single method for MRI-based analysis of liver function that is confirmed by active researches in this direction.
