The cavity shaving (CS) technique was described in breast conserving surgery to reduce the rate of reoperation avoiding the need for intraoperative margin analysis. This study assesses differences in the rates of involvement of the surgical margin (requiring further surgery) and volume of surgical specimens, depending on the use or not of this technique.
A retrospective cohort study was conducted in patients with breast carcinoma who underwent breast conserving surgery between 2013 and 2019. They were divided into two groups depending on whether the cavity shaving technique was used or not. Primary outcomes of the study included presence of final margin involvement, requiring need for further surgery, and the volume of excised tissue comparing the study groups.
A total of 202 cases were included 92 in the control group and 110 in the cavity shaving group. Significant differences were found regarding involvement of the final margin (19.57% control group vs. 4.55% cavity shaving group; p=0.010). The volume of additional surgical specimens were significantly greater in the traditional technique group than in the shaving technique (46.43 vs 13.32 cm
; p=0.01) as was total specimen volume (143.40 vs 100.63 cm
; p=0.022).
CS can reduce the positive margin and re-excision rates without larger-volume resections and should therefore be considered a routine technique in BCS for early-stage breast cancer.
CS can reduce the positive margin and re-excision rates without larger-volume resections and should therefore be considered a routine technique in BCS for early-stage breast cancer.We describe an extractionless real-time reverse transcriptase-PCR (rRT-PCR) protocol for SARS-CoV-2 nucleic acid detection using heat as an accurate cost-effective high-capacity solution to COVID-19 testing. We present the effect of temperature, transport media, rRT-PCR mastermixes and gene assays on SARS-CoV-2 gene amplification and limits of detection. Utilizing our heated methodology, our limits of detection were 12.5 and 1 genome copy/reaction for singleplex E- and N1-gene assays, respectively, and 1 genome copy/reaction by utilizing an E/N1 or Orf1ab/N1 multiplex assay combination. Using this approach, we detected up to 98% of COVID-19 positive patient samples analyzed in our various cohorts including a significant percentage of weak positives. Importantly, this extractionless approach will allow for >2-fold increase in testing capacity with existing instruments, circumvent the additional need for expensive extraction devices, provide the sensitivity needed for COVID-19 detection and significantly reduce the turn-around time of reporting COVID-19 test results.Skeletal muscles ensure the mobility of mammals and are complex natural fiber-matrix-composites with a hierarchical microstructure. In this work, we analyze the muscle's mechanical behavior on the level of fascicles and muscle fibers. We introduce continuum mechanics hyperelastic material models for the connective tissue endomysium and the embedded muscle fibers. The coupled electrical, chemical and mechanical processes taking place in activated contracting muscle fibers are captured including the temporal change of the activation level and the spatial propagation of the activation potential in fibers. In our model, we investigate the material behavior of fascicle, fiber and endomysium in the fiber direction and examine interactions between muscle fiber and endomysium by considering the temporal and spatial change of muscle fiber activation. In addition, a loading case of normal and shear forces is applied to analyze the fiber lifting force and the lifting height of unipennate muscles with different pennation angles. Moreover, the development of local stresses and strains in fibers and endomysium for different strains are studied. The simulation results allow to identify regions in high risk of damage. Optimal arrangements of unipennate muscle microstructure are found for either very small or very large pennation angles.

A cross-sectional survey.
Data were collected from nursing students in six provinces in different parts of China between September and October 2018.
Six hundred and ninety-four nursing students participated in this study.
Purposeful sampling was used, and students completed online survey to explore the structure of the CLES+T scale. Internal consistency reliability was tested on the total scale and its sub-dimensions using Cronbach's alpha.
An exploratory factorial analysis identified a three-component solution, with 11 items, and explained 60.01% of the variance. Confirmatory factor analysis supported the factor structure. Cronbach's alpha coefficient for the scale was 0.82, and those of the three subscales were 0.79, 0.71, and 0.70.
The unique, stable and relatively concise Chinese version of CLES+T scale instrument could be a useful instrument for using as an assessment scale in Chinese-speaking countries.
