Malassezia spp. are lipophilic yeasts implicated in the pathogenesis of chronic skin diseases. Repeated therapies are often necessary due to the recurrence of this type of disease. Recently, laser and light-based devices used for the treatment of some skin diseases have shown good efficacy, few contraindications, and minimal side effects. The neodymium-doped yttrium aluminium garnet (Q-switched NdYAG) laser is one of the most commonly used lasers in dermatology. The aim of this study was to evaluate the effect of the Q-switched NdYAG laser (Medlite C6 laser, Conbio, USA) on the pathogenic mechanisms of M. furfur during skin infections. Following laser exposure, the ability of M. furfur to retain phospholipase activity, upregulate the aryl receptor and its associated pathway, and stimulate the immune response were tested. The Q-switched NdYAG laser was shown to attenuate the virulence of M. furfur. The Q-switched NdYAG laser should be considered as a valid therapeutic alternative for the treatment of Malassezia-associated infections.Since direct oral anticoagulants have been marketed, the management of major bleeding in emergency departments has become more challenging. In 2013 and 2016, the Working Group in Perioperative Haemostasis made several proposals for this. The objective of the present study was to evaluate conformity with these proposals in daily clinical practice. We performed a retrospective single-center cohort study addressing the management of major bleeding in patients treated by direct oral anticoagulant in an emergency department. Patients were included between January 1, 2015 and November 30, 2017. Among 3,365 patients admitted to the emergency department for bleeding, a total of 191 were treated by direct oral anticoagulant and 55 (28.8%) with major bleeding were included in the present study. The majority of patients received rivaroxaban (n = 44, 80.0%) and were treated for atrial fibrillation (n = 47, 85.0%). Bleeding concerned gastrointestinal (n = 30, 54.6%) and (n = 14, 25.5%) intracranial location. Twenty-one patients (38.2%) were treated by prothrombin complex concentrate, and 31 patients (56.4%) were transfused. Intervention for hemostatic control was performed for 30 patients (54.6%). Thirty-three patients (60.0%) presented a deviation from protocol. There is no significant difference for mortality at 3 months according to whether or not there was a deviation from protocol. Nearly two-thirds of patients with major bleeding associated with direct oral anticoagulant administration had protocol deviation, but there was no effect on mortality.Skin cancer is the most common malignancy with rising incidence. Although early detection can be lifesaving, prevention programmes are under-utilized. In 2008, a group of board-certified dermatologists in Switzerland established a website aimed at educating about skin cancer risk factors and providing guidance on self-assessment. To present the data of this programme over the last 10 years with regards to representation of the targeted groups and sustained impact on primary skin cancer prevention. A comprehensive web-based health promotion campaign was established for education and guidance on self-assessment. Teledermatological evaluation was offered and participants were then interviewed. In total, 11,171 digital photos were evaluated during 2008-2018; 54.3% (n = 6,067) from females and 45.7% (n = 5,104) from males. In 26.7% (n = 2,983), clinical examination was recommended. Of the participants, 1,874 replied revealing 103 malignancies (9.2% of the lesions were presented to a physician) 34 melanomas in situ, six squamous cell carcinomas, 53 basal cell carcinomas and 10 malignant lesions (not further specified). Of the participants, 40.5% (n = 683) changed their attitude towards sun exposure, 48.7% (n = 820) used more sunscreen, and 57.5% (n = 966) improved sunscreen measures. Web-based educational programmes raise public awareness, enhance prevention, and support early diagnosis of skin cancer. Dementia with Lewy body (DLB) is a common neurodegenerative disease that warrants specific care, which remains largely underdiagnosed. Our objective was to assess the knowledge of DLB by health professionals in comparison with that of Alzheimer's disease (AD), to better understand the reasons of its under-diagnosis.

We conducted a descriptive and analytical study processing the results of an online questionnaire submitted to French healthcare professionals between December1, 2020 and March1, 2021.

A total of 490healthcare professionals responded to the questionnaire. We observed a poorer knowledge of DLB compared to AD both subjective as highlighted on the self-assessment questionnaires and objective since the diagnostic criteria and therapeutic specificities were less known for DLB compared to AD.

Yet, the emotional strain of the pandemic exposed the mental health needs of survivors and society as a result of forced changes in occupational choices, habits, and roles. These forced changes caused a collective confusion, fear, loss, and grief. This column focuses on occupational therapy's critical contributions to supporting society's mental health and wellness. Occupational therapy has a unique opportunity to provide interventions to address population-wide mental health and wellness, targeted prevention of mental health concerns for people at risk, and intensive intervention for people with COVID-19-induced mental health diagnoses.This column discusses issues in the delivery of postacute care (PAC) rehabilitation services for coronavirus disease 2019 (COVID-19) survivors and gaps in the current research. Occupational therapy practitioners must not only better understand factors that influence the type of PAC rehabilitation a COVID-19 survivor will receive but also comprehend how wide variations in delivery of PAC occupational therapy have affected important outcomes for survivors. The COVID-19 Rehabilitation Research Framework, developed by Cochrane Rehabilitation and the World Health Organization Rehabilitation Program, offers a guide for occupational therapy research priorities that may fill two important gaps (1) the need for high-quality PAC rehabilitation studies and (2) the need for research on activity and participation assessments and outcomes for COVID-19 survivors. The client was a 43-yr-old Hispanic man treated in a long-term acute-care hospital. Client-centered treatment sessions focused on activities of daily living (ADLs), self-feeding, medication management, and leisure and were implemented in 30- to 45-min sessions 3 times per week for 5 wk. The Activity Measure for Post Acute Care Daily Activities Short Form was used to measure basic mobility, daily activities, and applied cognition in the acute setting. A manual dynamometer was used to measure grip strength, and the Nine-Hole Peg Test was used to measure digit dexterity. Both were used in the context of occupational engagement. Data were collected at evaluation, every 2 wk, and at discharge. The client achieved his goals and demonstrated marked improvement in independence with basic ADLs, leisure activities, bilateral grip strength, and manual dexterity.
