Psychological disorders often take a long-term course, resulting in impairment in daily life and work. Treatment must therefore target not only symptoms of illness but also capacity limitations and context restrictions, as outlined in the International Classification of Functioning, Disability and Health (ICF). This includes sociomedical and interdisciplinary interventions like coordination with other specialists, contact to employers and employment agencies, social support agencies, debt counselling, self-help and leisure groups. There are no data on the spectrum, rate and unmet needs of sociomedical interventions in outpatient psychotherapy.

Following a semistructured interview schedule, 131 psychotherapists in private practice were asked to report on unselected patients. The interviewer assessed to what degree 38 predefined sociomedical interventions were applied so far or should be considered in the future.

Reports for 322 patients were gathered. All sociomedical interventions were applied, depending on the sick leave status and course of illness. Cognitive behaviour therapists used more sociomedical interventions than psychodynamic therapists.

The data show that sociomedical interventions are a frequent part of psychotherapy. They are used preferably in patients with participation restrictions. Psychotherapeutic concepts and education should include sociomedical aspects.

C-reactive protein (CRP) has been associated with adiposity and cardiometabolic disease risk in many populations but remains remarkably understudied in Pacific Islander populations. Here, we provide the first examination of correlates of CRP in adult Samoans (n=108, ages 35-55 years) to test the hypotheses that CRP exhibits sex-dependent associations with measures of BMI, adiposity, and cardiometabolic disease risks.

We analyzed associations between measures of adiposity (total fat mass, visceral fat mass, percent total body fat), body mass index (BMI), cardiometabolic risks, behaviors, demographics, and CRP. Unadjusted analyses of CRP were undertaken using Pearson's pairwise, and Spearman's rank correlations; one-way analysis of variance and Kruskal-Wallis tests assessed variables by CRP quartiles. Adjusted analyses of CRP correlates were examined using generalized linear regression.

Serum CRP ranged from 0.08 to 13.3mg/L (median 1.4mg/L) and varied significantly by sex t (108)=-2.47, p=.015. CRP was weakly to moderately associated with measures of adiposity and BMI (r and ρ ranged between 0.25 and 0.50, p < .05) and some cardiometabolic markers (including HbA1c, fasting insulin, and insulin resistance). CRP was significantly associated with percent body fat in women and men, adjusting for other variables.

These data are among the first to demonstrate CRP correlates in a sample of adult Samoans. CRP differed by sex and was associated with BMI, adiposity, and some cardiometabolic risk markers. These data align with findings in other populations.

Fruit juices are naturally acidic, and the acidity is due to the formation of various organic acids formed in several metabolic processes. The content of acids varies due to various processing parameters during the preparation of fruit juices and their packaging for commercialization. Quantitative determination of organic acids provides the necessary information leading to changes occurred during processing.

The organic acids were extracted by ion pair dispersive solid phase extraction by multiwalled carbon nanotubes and analyzed by gas chromatography-mass spectrometry and in-situ butylation.

The developed analytical method was validated and the obtained results showed linearity in the range of 0.5 to 5000 μg/L levels of the analytes with a limit of detection and quantification values of 2-10 μg/L and 5-20 μg/L, respectively. The inter and intraday reproducibility are less than 15 % with 80-98 % recoveries and less than 20 % matrix effect. The developed method was used for the quantitative determinationility for the quantitative determination of various organic acids in the fruit juices. Fumaric acid was observed as the major carboxylic acid present in the fruits. The processed juice samples possess lowest concentration of organic acids suggesting that processing has a significant effect on the concentration of organic acids in fruits.
Given its widely accepted efficacy, androgen blockade therapy for hidradenitis suppurativa (HS) has become a standard of care. Although much less frequently used than spironolactone, a small number of HS studies have reported finasteride as an alternative in women. In this study, we describe the response to and perception of finasteride therapy in a diverse cohort of women with HS.

We aimed to describe finasteride therapy in a diverse cohort of female patients with HS.

We conducted an IRB-approved retrospective chart review and telephone survey of 20 female patients, 18-years or older, with a diagnosis of HS. Finasteride was prescribed by a single provider at a specialized hidradenitis suppurativa center.

The mean age of patients was 34.3 ± 13.5 years. Finasteride was initiated predominantly because of one or more contraindications to spironolactone or poor responsiveness. Most patients interviewed (90%; n=18) were willing to take finasteride again or continue with therapy if indicated. Ten patients (50%) reported overall satisfaction with finasteride; seven (35%) were neutral, and three (15%) were unsatisfied. No patients reported worsening disease activity while on finasteride, and only one (5%) reported decreased quality of life. When asked about side effects of finasteride, 80% (n=16) reported none; 20% (n=4) experienced one or more of the following headache, nausea, menstrual irregularities, breast tenderness or reduced libido/sexual function.

The overall safety profile of TEE probe is quite favorable with a reported incidence of complications of 1%-3%. However, insertion of the TEE probe can induce vagal and sympathetic reflexes such as hypertension or hypotension, nonsustained ventricular and supraventricular tachyarrhythmias, or bradyarrhythmias (third degree heart block), and even angina and myocardial infarction. TRAF3IP2 interference inhibited HG-induced cell viability loss, cytotoxicity, inflammatory response, oxidative stress and apoptosis of the cardiomyocytes. Moreover, saxagliptin, empagliflozin and linagliptin inhibited the expression of TRAF3IP2. TRAF3IP2 interference alleviates HG-induced inflammation and apoptosis of cardiomyocytes. The result suggests that TRAF3IP2 may be a promising therapeutic target in treating diabetic cardiomyopathy.This study aimed to evaluate the efficacy and safety of vasal vessel-sparing modified single-armed 2-suture longitudinal intussusception vasoepididymostomy (SA-LIVE) to epididymal obstructive azoospermia patients. Forty consecutive epididymal obstructive azoospermia cases, who underwent microsurgical vasoepididymostomy in Shanghai General Hospital from January 2019 to October 2019, were included in this study. Twenty cases underwent SA-LIVE (group A), and 20 cases underwent vasal vessel-sparing SA-LIVE (group B). Until March 2021, the mean follow-up period was 16.9 ± 4.1 (12-23) months. The overall patency rate was 82.5%, and 80% and 85% for group A and group B respectively. The mean time to achieve patency was 4.11 ± 2.74 months. The overall natural pregnancy rate was 51.5%(17/33) at the mean follow-up of 16.9 months. The natural pregnancy rate was 50.0% for group A and 52.9% for group B (p > .05). At the time of 6 months post-operation, the patency rate was 70% for group A and 80% for group B (p = .465); the natural pregnancy rate was 0% for group A and 31.3% for group B (p = .022). Vasal vessel-sparing SA-LIVE is safe and effective to achieve favourable patency and pregnancy rates. Preserving vasal vessel would improve natural pregnancy rate at a very early stage.
