Retrospective analysis of prospectively collected clinical data from four community-based hospitals and one academic medical center. 8173 adults were included. Heart rate (HR) was used as a surrogate marker of sympathetic stimulation. HR, MAP, and lactate levels were measured upon presentation.

MAP and HR interacted to affect lactate levels with the highest levels observed in patients with low MAP and high HR (3.6mmol/L) and the lowest in patients with high MAP and low HR (2.2mmol/L). The overall mortality rate was 12.4%. Each 10 beats/min   less then  65 mmHg) and was a significant predictor of mortality. Because HR, lactate, and MAP were associated with mortality, our data support the present strategy of using these measurements to gauge severity of illness upon presentation. Since HR did not moderate or mediate the association between lactate and death, criticisms alleging that lactate caused by sympathetic stimulation is adaptive (i.e., less harmful) do not appear substantiated.To assess if radiomics can differentiate left atrial appendage (LAA) contrast-mixing artifacts and thrombi on early-phase CT angiography without the need for late-phase images. Our study included 111 patients who underwent early- and late-phase, contrast-enhanced cardiac CT. Of these, 79 patients had LAA filling defects from thrombus (n = 46, mean age 72  ±  12 years, MF 2620) or contrast-mixing artifact (n = 33, mean age 71  ±  13 years, MF 2112) on early-contrast-enhanced phase. The remaining 32 patients (mean age 66  ±  10 years, MF 1913) had homogeneous LAA opacification without filling defects. The entire LAA volume on early-phase CT images was manually segmented to obtain radiomic features (Frontier, Siemens). A radiologist assessed for the presence of LAA filling defects and recorded the size and mean CT attenuation (HU) of filling defects and normal LAA. The data were analyzed using multiple logistic regression with receiver operating characteristics area under the curve (AUC) as an output. The radiologist correctly identified all 32 patients without LAA filling defects, 42/46 LAA with thrombi, and 23/33 contrast mixing artifacts. Although HU of LAA thrombi and contrast mixing artifacts was significantly different, with the lowest AUC (0.66), it was inferior to both radiologist assessment and radiomics (p = 0.05). Combination of radiologist assessment and radiomics (AUC 0.92) was superior to HU (0.66), radiomics (0.85), and radiologist (0.80) alone (p  less then  0.008). Radiomics can differentiate between LAA filling defects from thrombi and contrast mixing artifacts on early-phase contrast-enhanced CT images without the need for late-phase CT.Left atrial (LA) mechanics assessed by two-dimensional speckle tracking echocardiography (2D-STE) have not been extensively studied and clearly characterized in patients with moderate aortic stenosis (AS). Accordingly, we aimed to evaluate the usefulness of LA reservoir strain for risk stratification of asymptomatic patients with moderate AS. This retrospective study included all consecutive asymptomatic patients with moderate AS who underwent transthoracic echocardiography implemented with 2D-STE analysis of LA myocardial strain and strain rate indices at our Institution, between February 2011 and September 2019. During the follow-up period, we evaluated the occurrence of any of the following (1) CV hospitalization; (2) The recourse to AS surgery; (3) Cardiac death or sudden death. A total of 186 patients (mean age 71.9 ± 12.7 years, 61.8% men) were included in the present study. During a mean follow-up of 2.3 ± 1.9 years, no patients died and 63 adverse CV events were recorded 48 patients were hospitalized because of heart failure (28 patients), acute coronary syndrome (10 patients), arrhythmias (10 patients) and 15 patients underwent AS surgery. At the multivariate Cox regression analysis, type 2 diabetes mellitus (OR 1.87, 95%CI 1.05-3.34, p = 0.03), NT-proBNP (OR average 1.14, 95%CI 1.02-1.27, p = 0.02), average E/e' ratio (OR 1.07, 95%CI 1.01-1.15, p = 0.04) and most of all left atrial positive global strain (LA-GSA+) (OR 0.85, 95%CI 0.81-0.90, p  less then  0.0001) were independently associated with the outcome. We investigated the effect of spironolactone on cardiac structure and function with a specific focus on diastolic function parameters. The MiREnDa trial examined the effect of 50 mg spironolactone once daily versus placebo on left ventricular mass index (LVMi) among 97 HD patients during 40 weeks of treatment. In this echocardiographic substudy, diastolic function was assessed using predefined structural and functional parameters including E/e'. Changes in the frequency of HFpEF were analysed using the comprehensive 'HFA-PEFF score'. Complete echocardiographic assessment was available in 65 individuals (59.5 ± 13.0 years, 21.5% female) with preserved left ventricular ejection fraction (LVEF > 50%). At baseline, mean E/e' was 15.2 ± 7.8 and 37 (56.9%) patients fulfilled the criteria of HFpEF according to the HFA-PEFF score.
