Avanos Medical Cortrak*2 Entry Access System was designed to aid health professionals in inserting medical feed tubes in the small bowel or stomach of patients who are requiring nutrition. There have been 23 deaths related to feeding tubes since 2015.
Avanos Medical feeding tube Patients have died and injuries because of the inadvertently placed nasogastric/naso devices.
This recall is used for updates to the device’s labeling and instructions for use. The users are advised to confirm that the tube has been placed according to the protocols of their establishment prior to applying it to provide nutrition.

This can lead to grave injury and possibly death.
Avanos Medical's recall notice stated that there were 60 injuries and 23 deaths linked to the misplacement or removal of nasogastric feed tubes in the usage of the CORTRAK*2 Entral Access System. This occurred in the year 2015. Affected events include respiratory failure and pneumothorax (collapsed lung), perforation (a hole in the wall of the lung, esophagus or bowel) as well as pneumonia (a lung infection) and pleural effusion (excess fluid that has accumulated in the lung space and the chest cavity).
Avanos Medical issued on March 21, 2022, a correction notice to customers who were using the devices between January 20th, 21st, and January 20th, 22nd. The notice provided the following guidelines:
Verify the location of Nasogastric tubes and the nasoenteric tube according to institution policies.
Attach the correction notice from the field describing the issue to the operator's manual.
