Avanos Medical Cortrak*2 Ental Access System was designed to allow trained health professionals to place medical feeding tubes inside the stomachs or small bowels of patients who require nutritional support. Patients have been killed and injured because of the incorrect placement of nasoenteric/nasogastric tube devices.
The recall will be used to update the information on the device's label along with the directions and intended uses. The new instructions advise users to verify the tube's placement in accordance with the guidelines of their institution prior to using the tube for delivering nutritional supplements.

Avanos Medical Avanos Medical recalls that there have been sixty accidents and 23 deaths due to misplacement in nasogastric tube placements in the last year. Avanos Medical, on March 21, 2022, issued a corrective field notice to customers who used these products between January 2021 and January 2022. The notice included the following instructions:
Verify the placement of nasogastric/nasoenteric in accordance to institutional policies
Attach the correction notice from the field describing the issue to the operator's manual.
