Avanos Medical and FDA issued a Class 1 recall of 629 CORTRAK*2 enteral access system, which is a feeding tube positioning device that has been implicated in numerous accidents and deaths.

Cortrak 2 eternal access system feeding tube placement Incorrect placement of nasogastric and Nasoenteric tube was the reason for 23 deaths and 60 injuries in the past year.
The recall will modify the label of the device so that it can remind users to check that the tube is correctly placed before delivering nutrition.
According to FDA the adverse events were respiratory failure, pneumothorax (collapsed lung) Perforation (a hole in the wall or bowel of the lungas well as pneumonia (a lung infection) and the pleural effusion (excess liquid in the lung space and your chest cavity).
