Avanos Medical faces a Class I recall of its feeding tube system connected to 23 deaths in 2015

Seven years after bad luck with Avanos Medical’s insertal feeding tube placement device for children, the FDA has issued its most serious warning regarding the recall of the device.

Avanos Medical feeding tube Cortrak*2 Enteral Access System has an electromagnetic stylet and an external receiver. It also includes an LCD monitor. The system can be combined to provide a live feed that illustrates the process of inserting a medical feed tube into the stomach of an individual. This procedure is carried out with the aim of improving accuracy and reducing complications.

Avanos was not pleased with the results and announced an immediate recall of all Cortrak*2 units between January 2021 & January 2022. The recall affected nearly 630 devices that were distributed between April 2016 & the beginning this year.

The safety incident doesn't count as a recall in any strict sense: Avanos doesn't ask healthcare professionals to request that the device be returned to the maker. It's merely to ensure that they're using the device in a safe manner.

A feeding tube installed incorrectly could cause injuries to vocal cords or the tracheas. Cortrak 2 eternal access system Avanos has reported more than 60 injury cases in 2015 according to the FDA. This was all due to the FDA's Cortrak*2 system, which directs the placement of feeding tubes.

A variety of injuries have been reported , including respiratory failure (collapsed lung) lung infection, pleural effusion, holes in the walls, esophagus and the bowel.

The FDA issued a march 21 field correction notice in which the Georgia-based firm urged users to confirm the placement of nasogastric tubes as well as nasoenteric pipes according to institutional policies. Avanos has requested that users add a safety warning to their operating manual and confirm that they have received the latest.

The FDA has issued a second caution regarding the use of an enteral tube for feeding use. Cortrak 2 eternal access system The FDA issued a safety communication in February that warned parents, healthcare professionals, and children of the risk of strangulation from tubes feeding food.

Following two deaths in 2021 the notification was issued. In both instances the tubing system was discovered to accidentally wrap around the neck of a patient who was less than two years old even though they were not being observed by their medical staff or caregivers.

"While FDA believes that death from strangulation using tubing for feeding tubes used in the enteral system for children is rare, it's important that caregivers and healthcare providers are aware that these events could and do occur," the FDA stated in the announcement.
