The Avanos Medical Cortrak 2 Entry Access Systems is designed to aid trained health professionals in inserting medical feeding tubes into the small bowel or stomach of patients who require nutrition. Avanos Medical Since 2015, however 23 patients have died due to complications related to feeding tube.
Avanos Medical Patients have suffered fatalities and injuries as a result of misplacement of nasoenteric/nasogastric tube placement devices.

The updated instructions instruct users to verify the tube's placement according to the protocols of their institution prior to using the tube to deliver nutritional supplements.

Avanos Medical has issued a recall communication stating that since 2015, there have been 60 patient deaths and 23 injuries resulting from an incorrect placement of the nasogastric feeding tube. Affected events include respiratory failure, pneumothorax (collapsed lung) perforation (a hole in the lung's wall, esophagus, or bowel), pneumonia (a lung infection) and the pleural effusion (excess fluid in the space between the lungs and chest cavity).
Avanos Medical, on March 21, 2022, issued a field correct notification to its customers who purchased these products between January 2021-January 2022. The following guidelines were included in the notice.
Check the positioning of nasogastric and the nasoenteric tube according to institution policies.
