Avanos Medical is recalling Cortrak*2 Entry Access System. Avanos offers the Cortrak*2 enteral access device for its visualization of the patient's small bowel and stomach, which helps in placing a medical feeding tube. However, patients could suffer from damage to their vocal cords and lungs if the tube is not properly inserted. Cortrak 2 eternal access system Avanos Medical released a recall statement stating that there had been a total of 60 incidents and 23 deaths caused by mistaken placement, in 2015, because of the CORTRAK*2 Entry Access System. Avanos Medical describes a plan to update labeling to make sure that the Nasogastric or Nasoenteric tubes prior to it is used.
Avanos was the second caution issued by the FDA in 2022 concerning tube feeding through the stomach. In February, parents and healthcare professionals were informed by the FDA about the potential for strangulation of children using tubes for feeding. Two deaths of children from strangulation were reported in 2021. as a result of a tube system that wrapped around a neck of a patient.
