Avanos Medical pulled Cortrak*2 EAS off service following reports about injuries and deaths related to tube missing.
The US Food and Drug Administration has acknowledged the Cortrak*2 of Avanos Medical's Enal Access System Recall (EAS) as a Class I recall.

Avanos Medical called off Cortrak*2, EAS, due to deaths and injuries resulting from misplacements of nasogastric tubs.
The device was designed to assist health care professionals when placing medical feed tubes into patients.
But, improperly placed nasogastric/nasoenteric pipes could cause harm to the vocal cords and the trachea. This could lead to serious injuries or even death.
The company has recalled 629 devices throughout the US between January 2016 to January 2022.

Avanos Medical will revise the labelling after the recall. This includes updating the instructions and intended uses for Cortrak* 2 EAS.
The updated guidelines instruct users to ensure that the tube has been installed in accordance with the guidelines of their institution prior to use.
