Avanos Medical and FDA issued a Class 1 recall of 629 units of CORTRAK*2 an enteral access device, which is a feeding tube placement device that has been linked to numerous incidents of injury and death.

Incorrect placement of the nasogastric or Nasoenteric tube is responsible for the deaths of 23 and injuries to 60 in the last year.
This recall will be used to correct the label on the device, in order to remind users to check that the tube is correctly put in place before they could deliver nutrients.
According to FDA, "Adverse events include respiratory failure, pneumothorax, perforation (a hole or puncture in the lung wall, esophagus, or the bowel) as well as pneumonia, lung infection and pleural effusion ("excess fluid between the lungs and chest cavity),"
