According to the U.S. Food & Drug Administration (FDA), Avanos Medical is recalling the Cortrak*2 Enteral Access System due to the incorrect placement of nasogastric and nasoenteric tubes has led to injury and death of patients. The recall currently includes 629 devices that were distributed across the United States.

Avanos Medical Patients can suffer from injuries to their vocal chords or trachea, if the tube is placed incorrectly. According to Avanos Medical's recall announcement there have been 60 injuries and 23 deaths in patients in 2015 due to the misplacement of feeding tubes for the nasogastric system making use of the CORTRAK* 2 Enteral Access System. Pneumothorax, pulmonary failure (collapsed lungs) perforation (a hole in one's stomach or bowel wall) pneumonia, an excess of fluid between the lungs (the chest cavity and the lung tissue) have all been among the injuries.

Avanos is the second warning issued by the FDA has issued about intra-oral feeding tubes. The FDA issued a caution to parents and healthcare providers regarding the danger of strangulation when feeding tubes are being used by children. Two cases of child strangulation occurred in 2021. This was due to a tubing system placed around the neck of a patient.
