Avanos Medical feeding tube This recall was identified by the FDA as a Class I recall. It is the most serious type of recall. Use of these devices may result in serious injuries or even death.

The FDA has identified Avanos Medical as the person responsible for recalling their Cortrak*2 system for enteral access. 629 devices, which were distributed between 2016 and 2022, were recalls by the U.S. starting on March 21.
Cortrak*2 helps clinicians place medical feeding tubes inside the stomach or in the small bowel of patients needing to receive nutrition via the tube.
Avanos Medical The recall was brought on by death and injury reports of patients suffering from nasocentric/nasogastric tube mistaken placement. Avanos Mediacal Cortrak 2 The device is used to assist in placing the tubes. Avanos Medical feeding tube A wrongly placed nasogastric or nasal tube could cause severe injury or death.
Avanos Medical feeding tube According to Avanos Safety communications, there were 60 injuries and 23 deaths attributed to this incident. Pneumothorax (perforation), pneumothorax (perforation) and pneumonia and pleural effusion were all identified as adverse events.
Cortrak*2 has used this recall to update the instructions for its usage and intended use. Users must confirm the placement of tubes in accordance with established protocols before they can deliver nutrition.
Clinicians were also encouraged to attach the corrected field notice on the problem to Avanos' operating manual, and to return the acknowledgement forms that came in Avanos' notice. Avanos Mediacal Cortrak 2 The company plans to offer users current labeling, which will include confirmation of tube placement in accordance with the institution's policies prior to use.
