Avanos Medical and the FDA have issued a Class 1 recall for about 629 units of the CORTRAK*2 Entral Access System, a feeding tube placement device linked to dozens of deaths and injuries.

Incorrect placement of nasogastric as well as Nasoenteric tubes has been the reason for 23 deaths and 60 injuries in the last year.
Avanos Mediacal Cortrak 2 The recall will update the label of the device to make it clear to users that the tube is correctly placed prior to feeding.
"Adverse incidents reported include respiratory failure and pneumothorax (collapsed lung) and perforation (a hole in the wall of the lung the esophagus, bowel, or stomach) or pneumonia (a lung infection) and the pleural effusion (excess fluid in the lung cavity and the chest cavity)," according to the FDA.
