The Avanos Medical Cortrak*2 Entry Access Systems is designed to aid health care personnel in inserting medical feed tubes in the small bowel or stomach of patients who require nutrition. There were 23 deaths attributed to the feeding tube in 2015.
Patients have suffered fatalities and injuries as a result of misplacement of nasoenteric/nasogastric tube placement devices.

The new instructions advise users to verify the tube's placement in accordance with the guidelines of their institution before using the tube for delivering nutrition.
The vocal cords, lungs and the trachea may be damaged when the nasogastric/nasoenteric tube is installed improperly. This could result in severe injury, or even death.
Avanos Medical's recall announcements stated that there have been 60 injuries, 23 deaths, and 58 patients who were injured due to the incorrect placement of Nasogastric tubes.
