Avanos Medical faces a Class I recall for its the feeding tube system linked to 23 deaths that occurred in 2015

After seven years' of failure in Avanos Medical, Avanos Medical’s Enteral Feeding Tube Placement System, the FDA has issued the most serious recall notice ever issued.

Despite that goal it has been implicated in the occurrence of numerous injuries to patients which led Avanos to issue a recall earlier this year of all Cortrak*2 devices between January 2021 to January 2022. This totals around 630 devices initially distributed between April 2016 until the beginning of this year.

A tube for feeding that is not properly inserted could result in serious injury or death. Avanos was notified of more than 60 injuries and 23 deaths in 2015 by the FDA. These incidents were all connected to the Cortrak*2 system, which guides the positioning of feed tubes.

The FDA reminded users of the need to verify the placement of a nasogastric or nasoente tube according to their institutional guidelines in the March 21 field correction notification. Avanos has asked patients to attach a safety notice to their operating manuals and verify that they've been updated.

Avanos stated that it will soon issue updated labeling for its product. This will include directions for locating the position of a tube in accordance to their facility's policies.

The FDA has issued a second caution regarding the use of an enteral tube for feeding use. In February, the FDA issued a safety announcement warning parents and health professionals of the possibility of strangulation as a result of the usage of tubes for feeding for children.

In the wake of two deaths that occurred in 2021, the notification was sent. A tubing system was not monitored by staff or caregivers placed around the necks and necks of infants under two years of age.
