The FDA has confirmed Avanos Medical's recall of its Cortrak*2 intramuscular access system. 629 devices, distributed between 2016 and 2022, were recalled by U.S. starting on March 21.
feeding tube placement Cortrak*2 aids clinicians when placing medical feeding tubes into the stomachs or small bowels of patients who need to be fed via the tube.
The recall was triggered by death and injury reports from patients after nasocentric/nasogastric tube mistaken placement. This device is utilized to aid in the placement of these tubes. Avanos Medical feeding tube If the tube is not properly placed, it could cause severe injury or death.

Clinicians were also advised to attach the field correction notice to the operating manual , and return the acknowledgement forms included with the notice to Avanos. Users will soon receive updated labels and confirmation that tubes were placed according to institution policy.
