The FDA has discovered the recall by Avanos Medical of Cortrak*2 access system for enteral use. 629 devices, distributed between 2016 and 2022 were recalled by U.S. starting on March 21.

Avanos Medical According to Avanos the number of incidents has been more than 60 injuries and 23 deaths related to this issue. Pneumothorax, pneumothorax (perforation) and pneumonia and pleural effusion have all been reported as adverse events.
Avanos Medical Cortrak*2 has used this recall to improve the guidelines for usage and intended use. Users will need to confirm the proper placement of tubes based on established protocols before they can deliver nutrition.
Clinicians were also urged to attach the corrected field notice on the problem to Avanos operating manual and to return the acknowledgment forms included with Avanos' announcement. Avanos plans to give users an updated labeling, including confirmation of tube placement according to institutional policies before usage.
