Avanos Medical (U.S. Food and Drug Administration) has announced that it will be recalling Cortrak*2's Enteral access system. The recall was announced in response to injuries sustained by patients and deaths resulting from the misplacement of nasoenteric/n tubes. Cortrak 2 eternal access system Avanos Medical Avanos has marketed the Cortrak*2 Enteral Access System for its screen-based visualization of a patient's stomach , or small bowel, to aid in the proper positioning of a medical feed tube. Patients may suffer injuries to their vocal chords, lungs or the trachea if the tube is not correctly placed. Avanos Medical's recall notice states that as of 2015, there have been 60 patient deaths and 23 injuries due to misplacement nasogastric feed tubes when using the CORTRAK*2 entry access system. The injuries include pneumothorax, respiratory failure, and perforation. These are all forms of lung collapse.

Avanos was the second alert issued by the FDA in 2022 concerning tube feeding through the stomach. In February, parents and health professionals were informed by the FDA about the potential for strangulation of children with tubes for feeding. Two child strangulation deaths were reported in 2021. the result of a tubing system that wrapped around a neck of a patient.
