The FDA has identified Avanos Medical’s recall of their Cortrak*2 intramuscular access device. 629 of the devices produced between 2016 until 2022 were recalls, with the first recall occurring on the 21st of March.
Cortrak*2 helps clinicians place medical feeding tubes inside the stomach or small bowel of patients needing to receive nutrition via tube.
The system's recall was caused by the deaths and injuries of patients resulting from the incorrect placement of nasocentric/nasogastric tubes. Cortrak*2 will use this recall to update the instructions for its usage and intended use. The users will have to confirm the placement of tubes in accordance with institutional protocols, before they can be used to provide nutrition.
Clinicians were also asked to attach the notice of correction concerning the issue to the operating book and then return acknowledgment forms along with the notice to Avanos. The company is expected to be able to provide updated labels to patients, as well as confirmation of tube placement in accordance with the institutional guidelines.
