Avanos Medical Cortrak*2 Enteral Access System was developed to assist healthcare professionals with training to insert medical feeding tubes in the small bowel or stomach of patients who need to receive nutrition through the tube. 23 deaths have been reported to date due to the feeding tubes since 2015.
There have been numerous incidents of death and injuries in patients who have been misplaced nasogastric tubes.
This recall will make changes to the labeling of the device, which includes the usage instructions and intended uses. The new instructions tell users to confirm the placement of the tube as per the guidelines of their institution prior to making use of them to deliver nutritional supplements.

Verify the location of Nasogastric tubes and the nasoenteric tube in accordance with the institution's policies.
Attach the pertinent note of correction to the field in the operator's guide.
