Avanos Medical Announces Recalls of the Cortrak*2 Entry Access System
According to the U.S. Food & Drug Administration (FDA), Avanos Medical is recalling the Cortrak*2 Enteral Access System due to misplacement of nasoenteric or nasogastric tubes has led to accidents and deaths of patients. https://independent.academia.edu/MosleyEmborg The recall includes 629 devices distributed in the United States.

If the tube is not placed correctly, it can cause harm to vocal cords, lungs , and trachea. Avanos Medical Avanos Medical released a recall statement stating that there had been sixty accidents and 23 deaths resulting from mistaken placement, in 2015, because of the CORTRAK*2 Entry Access System. These injuries include respiratory failure, pneumothorax (collapsed lung) and perforation (a hole in the wall of the lung, esophagus, or the bowel) as well as pneumonia. the pleural effusion (excess fluid in the chest cavity and the lungs).

Avanos Medical described a plan for patients to receive a new labeling in order to confirm proper use of the nasogastric tube and nasoenteric pipe prior to use.
