The FDA has confirmed the recall by Avanos Medical of its Cortrak*2 entry access system. 629 devices, distributed from 2016 - 2022, were recalls by the U.S. starting on March 21.

The system's recall was due to patient deaths and injury following the incorrect placement of nasocentric/nasogastric tubes. These tubes are used to aid in placing them. A mistake in the placement of the Nasogastric or Nasoenteric tube could cause severe injury or even death.
According to Avanos the number of incidents has been an estimated sixty injuries and 23 deaths in connection with this issue. The adverse events that were reported include pneumothorax, respiratory failure, perforation, pneumonia, as well as pleural effusion.
Cortrak*2 has used the recall to make updates to its instructions to use. The updated guidelines will contain instructions for users to verify that tubes are placed in accordance with institution protocols before making use of them to deliver food.
Avanos Medical Clinicians were advised to attach the corrective notice for field use concerning the issue to their operating manual and to return the acknowledgement form which was enclosed with the notice to Avanos.
