According to the U.S. Food & Drug Administration (FDA), Avanos Medical recalls the Cortrak*2 Enteral Access System because the misplacement of nasogastric or nasoente tubes has led to injury and death of patients. Avanos Medical Avanos offers the Cortrak*2 enteral access system for its ability to visualize the patient's small bowel and stomach, which aids in placing a medical feeding tube. Avanos Medical If the tube is placed incorrectly, however the patient could suffer damage to their vocal cords, lungs, or the trachea. Avanos Medical has reported that there have already been sixty-three injuries and 23 deaths caused by misplacement of nasogastric feeding tubes using CORTRAK*2's Ental Access System. The injuries can be described as pneumothorax, respiratory failure, and perforation. These are all forms of lung collapse.

Avanos is the FDA's second caution about inserting feeding tubes. In February, parents and health professionals were advised by the FDA about the possibility of strangulation of children with tubes for feeding. Two deaths of children from strangulation were reported in 2021. as a result of a tube system that wrapped around a patient's neck.
