Avanos Medical (U.S. Food & Drug Administration) has announced it will recall Cortrak*2's Enteral Access System. The recall was initiated in response to injuries sustained by patients and deaths caused by misplaced nasoenteric/nasoga tubes. The recall covers 629 devices distributed in the United States.

According to the Avanos Medical recall notice it has been reported that there have been 60 injuries and 23 patient deaths since 2015 related to misplacement of feeding tubes for nasogastric patients making use of the CORTRAK* 2 Enteral Access System. Pneumothorax, pulmonary failure (collapsed lungs) and perforation (a hole in the esophagus or bowel wall), pneumonia, and excess fluid between the lungs (the chest cavity as well as the lungs) are all among the injuries.
