The Avanos Medical Cortrak*2 access system was created to permit trained health care professionals to place medical feeding tubes in the small intestine or stomach of patients who require nutrition. 23 deaths have been reported up to date due to feeding tubes since 2015.

The recall will be used to modify the labels on the device, including the instructions and intended uses. It is recommended that users verify that the tube is set according to the protocols of their institute prior to using it to deliver nutrition.

Affected events include respiratory failure and pneumothorax (collapsed lung), perforation (a hole in the wall of the lung or esophagus), pneumonia (a lung infection), and an effusion of the pleura (excess fluid that has accumulated in the lung space and the chest cavity).
Avanos Medical issued on March 21, 2022, a correction notice to its customers using these devices between January 20,21 and January 20,22. The notice provided the following information:
Place nasogastric or nasoenteric tubs in accordance with the institution's policies.
Attach the correction notice from the field that explains the problem to the operator’s manual.
