The Avanos Medical Cortrak*2 Ental Access System was developed to aid trained healthcare professionals to insert medical feeding tubes in patients' small intestines or stomachs. There have been 23 deaths reported to date due to the feeding tubes in 2015.
There have been substantive reports of injuries and patient deaths that have occurred when nasogastric tubes have been misplaced when this device is utilized to assist in their place.
This recall is utilized to make changes to the device's labeling that include the directions on how to use it. The update instructs users to ensure that the tube is set up according to the institution's guidelines prior to using the tube for nutrition delivery.
Patients may suffer serious injuries or even death in the event that a nasogastric tube, or nasoenteric tub has been incorrectly placed.
Avanos Medical has issued a recall notification stating that, since 2015, there were 60 patient deaths and 23 injuries due to misplacement nasogastric feeding tube. Avanos Medical, on March 21, 2022, sent a corrective field notice to customers who used these products between January 2021-January 2022. The notice provided the following information:
Use nasogastric or Nasoenteric tubs according to institution policies.
Add the notice of correction for field regarding the issue to the operator's guide.
