The FDA has identified Avanos Medical's recall of its Cortrak*2 useal access system. 629 devices that were distributed between 2016 and 2022 were recalled within the U.S., beginning on the 21st of March.
Avanos Medical Cortrak*2 helps clinicians place medical feeding tubes inside the stomach or in the small bowels of patients needing to receive nutrition through the tube.
The system's recall was caused by injuries and deaths of patients due to an error in placement of the nasocentric or nasogas. feeding tube placement These tubes assist in placing them. Nasogastric, also known as nasal, tube which is not properly placed could result in damage to the vocal chords and the trachea. This could lead to serious injuries, or death.
According to Avanos Safety communications there were 60 injuries and 23 deaths attributed to this issue. There have been several adverse events that occurred, including respiratory failure, pneumothorax and perforation as well as pleural effusion and pneumonia.
Cortrak*2 is using this recall to update its instructions for usage and intended use. Users will need to verify the location of tubes based on established protocols before they are able to deliver nutritional supplements.
Clinicians were also urged to add the updated note in the field on the problem to Avanos operating manual and return the acknowledgement forms with Avanos' notice. Users will soon receive updated labeling and confirmation that the tubes were placed in accordance with the institution's guidelines.
