The FDA has identified Avanos Medical as the company responsible for recalling its Cortrak*2 system of access to the enteral. 629 devices made between 2016 to 2022 were recalled with the first recall occurring on March 21.
Cortrak*2 is a device that allows doctors to insert medical feeding tubes in patients' stomachs or small bowels.
In the wake of deaths and injuries due to injuries and deaths, the system was recall. A nasogastric, or the nasoenteric tube, that isn't properly inserted can cause damage to the vocal chords and trachea. It could result in serious injuries, or death.
According to Avanos the safety communication, there were 60 injuries and 23 fatalities related to this incident. Adverse events reported included pneumothorax, respiratory failure perforation, pneumonia and the development of pleural effusion.

Clinicians were urged to attach the corrective field notice concerning the issue to their operating manuals and return the acknowledgement form that was enclosed with the notice to Avanos.
