Avanos Recalls FDA Class II Following 23 deaths attributed to the Device
Avanos Medical Cortrak*2 Entry Access System is designed to aid health care personnel in inserting medical feed tubes in the small bowel or stomach of patients who require nutrition. Since 2015, there has been 23 deaths related to feeding tubes.

There have been many reports of injury and death among patients who been misplaced nasogastric tubes.

Patients may suffer serious injuries or even die if a nasogastric tube or nasoenteric tub is incorrectly placed.

Avanos Medical feeding tube Avanos Medical recalls that there was a total of 60 accidents and 23 deaths due to mistakes in the placement of nasogastric tubes in the last year. Respiratory failure, pneumothorax perforation (a hole in the stomach stomach or wall) and pneumonia were all reported.

Avanos Mediacal Cortrak 2 Avanos Medical, on March 21, 2022, sent an correct field notice to customers who had used the products between January 2021-January 2022. The following guidelines were included in the notification.

Confirm placement of the nasogastric and nasoenteric and other tubes in accordance with the policies of the institution.
