The most deadly FDA Recalls 2022. Misplaced Feeding Tubes The most feared (Avanos Medical)
Avanos Medical Cortrak2 feeding tubes are among the most hazardous medical devices as per FDA recalls.

Avanos Medical feeding tube The year 2022 hasn't come to an end, but the FDA's list of recalled medical devices is already at 50. This recall has caused the tragic deaths of 36 and 224 injuries. Avanos Medical is first on the list for 2022 regarding medical device malfunctions, with 23 reported deaths due to misplaced feeding tubes.


These are the four most hazardous medical device errors, in accordance with the FDA recall notification:

Avanos Medical Recalls Cortrak*2 Ental Access System for Feeding Tubes
Inadvertently removing the nasogastric feeding tube tubes resulted in 60 injuries and 23 deaths.

In 2022 the year 2022, this malfunction was the leading cause of death for all the medical equipment that was recalled.


Avanos Medical has recalled the Cortrak*2 Enteral Access System due to reports of injuries and patient death after nasoenteric feeding tubes have been placed incorrectly.

As a precautionary measure, Avanos Medical sent notice to providers asking the hospital or user "...confirm the position of the NG/NI tubes per institutional protocol" Fox news reported.

Avanos Medical reported in a recall communication that deaths and injuries were caused by the improper placement or misuse of the feed tubes to the stomach while using the CORTRAK*2 Ental Access System.

Baxter recalls SIGMA Spectrum Infusion Pumps equipped with Master Drug Library (Version 8) and Spectrum IQ Infusion Systems with Dose IQ Safety Software (Version 9)
Baxter received 51 complaints of serious injuries as well as three reports of death of patients in the past five years.

Avanos Mediacal Cortrak 2 The medical device manufacturer Baxter has recalled the device following numerous reports of a safety alarm malfunction. In the event of an obstruction in the upstream the alarm did not trigger on the pumps. Warnings were made that these items could have negative health effects and even death.

Avanos Medical Medtronic recalls EMG reinforced Endotracheal Tube
There have been three injuries and 2 deaths associated with the use of this device.

The NIM CONTACT Reinforced EMG Endotracheal Tube and the NIM Standard Reinforced EMG Endotracheal Tube - which are used to help provide airway as well as monitor laryngeal nerves in head and neck surgeries - have been the source of three accidents and two deaths in the lead up to Medtronic's recall. While Medtronic hasn't asked clients to return the defective devices, the company released safety announcements in order to prevent the silicone cuff from blocking the airway of a patient.


If the tube is unable to ventilate properly or obstructs the airway, patients might be afflicted with oxygen deprivation and brain damage or death.

Baxter Healthcare Corporation Recalls Volara System
There was one incident of injury and 2 deaths associated with the use of this device.

Baxter Healthcare Corporation and its subsidiary company Hillrom are recalling the Volara System due to the fact that an in-line ventilation adaptor may prevent home-use patients not getting enough oxygen through their ventilators. There are some risks for those affected that include choking on mucus and other airway secretions and lung infection (pneumonia) that prevents oxygen from reaching the blood (respiratory Failure), brain injury caused by a lack of oxygen (hypoxia) along with death.
