Immersive Portals is an mod that was first released in the year 2019, however it has been updated frequently since. The mod relies on Fabric API, and it adds a few nifty features. The most obvious is the ability to actually look through portals, making the whole thing feel more connected. But, you can also design your portals to be horizontal or use different designs.
