Avanos Medical, FDA and FDA and FDA have issued a Class 1 Recall for about 629 units of CORTRAK*2's Ental Access System. This feeding tube placement device has been linked to many deaths and injuries.

Since 2015, there's been 20 deaths and 60 injuries recorded due to nasogastric and nasoenteric tubes that were not placed correctly.
The recall is used to alter the device's label to instruct users to make sure that the tube was placed correctly before delivering nutrition.
According to FDA, adverse events included respiratory failure as well as pneumothorax (collapsed lung) perforation (a hole in the bowel or wall of the lungand pneumonia (a lung infection) and the pleural effusion (excess liquid in the space between your lungs and your chest cavity).
